摘 要:目的探讨老年人低骨密度与冠心病的相关性,为临床冠心病的防治提供新的途径。方法选取213例老年患者进行选择性冠状动脉造影。根据造影结果将患者分为冠心病组和对照组,对所有患者的腰椎(L_([1-4]))及左侧股骨颈进行骨密度(BMD)测定;同时测定两组患者的年龄、BMI、血压、血钙、血磷、总胆固醇、甘油三酯、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇等指标,并采用多因素非条件logistic回归分析低骨密度与冠心病的相关性。结果两组患者的L_([1-4])及左侧股骨颈BMD值的差异均有统计学差异(均P<0.05),且冠心病组BMD值低于对照组。同时冠心病组的低骨密度(包括骨量减少和骨质疏松)发生率明显高于对照组,差异有统计学意义(P<0.05)。logistic回归分析显示低骨密度与冠心病存在明显的相关性。结论老年人低骨密度与冠心病之间存在相关性,是冠心病的独立预测因素。Objective To investigate the correlation between the elderly low bone mineral density (BMD) and coronary heart disease (CHD), to provide new ideas for clinical prevention and treatment of coronary heart disease. Methods 213 elderly patients underwent selective coronary angiography were included in the study, male 104 cases, female 109 cases. According to the result of angiography, the patients were divided into CHD group and control group, and al patients underwent BMD determi-nation in the lumbar spine(L1-4) and the left femoral neck. According to the results, the value of BMD of two groups were compared and analyzed with age, body mass index, blood pressure, serum calcium, phosphorus, total cholesterol,triglyceride, low density lipoprotein cholesterol, high density lipoprotein cholesterol index and other indicators. Multi- factor unconditioned logistic regres-sion was used to analyze the correlation between BMD and CHD. Results The differences of the CHD group and control group in lumbar spine(L1-4) BMD and left femoral neck (BMD) were statistically significant(P〈0.05), BMD in patients with CHD is lower than that of control group. At the same time the CHD group&#39;s low bone density(including osteopenia and osteoporosis) was higher than that of control group, there were statistical y significant differences (P〈0.05).Logistic regression analysis demonstrated that there were significant correlation between low BMD and CHD. Conclusion There is a correlation between the elderly low BMD and CHD, and it is an independent predictors of CHD.
