摘  要:目的:比较激光多普勒成像(LDI)技术和纸模法对皮瓣成活面积的测量结果,分析LDI测量皮瓣成活面积的组织病理学依据,评价该方法测量的准确性。方法:12只雄性SD大鼠,麻醉后形成以右侧腹壁下浅动脉为蒂的扩大腹部皮瓣,用显微血管夹阻断血供3h后恢复血供。术后第5天以LDI系统和纸模法行皮瓣成活面积测量,并在成活皮瓣边缘取材,用HE和免疫化学染色进行组织病理学分析。结果:LDI和纸模法测量的皮瓣成活面积分别为(10.19±3.27)cm2和(10.21±3.15)cm2,差异无统计学意义(P>0.05)。成活皮瓣具有正常皮肤组织结构和血管形态,而坏死皮瓣表现为炎细胞浸润、局灶性坏死、血管扩张和闭塞。结论:应用LDI技术测量微循环血流灌注可确定皮瓣成活情况,获得较准确的皮瓣成活面积数据。Objective:To determine the reliability of skin flap survival area results obtained with laser Doppler imaging(LDI)by comparing the results obtained with laser Doppler imaging or transparent paper and analyzing the pathologic results.Method:12male SD rats were used in this experiment.An extended epigastric adipocutaneous flap was raised,based on the right superficial epigastric artery and vein.3-hours flap ischemia was induced by clamping the pedicle vessels with microvascular clamp.On the fifth postoperative day,the survival area of the flap was recorded with LDI and transparent sheet.Then the rats were sacrificed and skin flap tissue was sampled at the edge of the survival area.The tissue was analyzed with HE and CD34staining.Results:The survival area obtained with LDI was(10.19±3.27)cm2.This area was(10.21±3.15)cm2if the flap was calculated with transparent sheet.The difference was not obvious(P>0.05).Skin tissue kept normal in survival area.Obvious inflammatory was observed in necrosis area of the flap.Micro-vessels in this area was dilated and jammed.Conclusion:Skin flap survival can be analyzed with LDI.Survival area results obtained with LDI are reliable.
