摘 要:目的:观察银杏叶提取物联合舒洛地特治疗糖尿病肾病(DN)Ⅲ期患者的临床疗效。方法:128例DNⅢ期患者,按随机数字表分为联合治疗组、银杏叶组、舒洛地特组和安慰剂组(对照组)。在常规治疗基础上,联合治疗组采用银杏叶提取物联合舒洛地特治疗,银杏叶组采用银杏叶提取物治疗,舒洛地特组采用舒洛地特治疗,对照组仅用安慰剂。连续两疗程。观察患者治疗前后血肌酐(Scr)、血尿素氮(BUN)、尿白蛋白排泄率(UAER)等肾功能指标以及全血表观黏度、血浆黏度、红细胞聚集指数、血浆纤维蛋白原(FIB)、红细胞压积(HCT)等血流变指标变化。结果:治疗前后,各组Scr、BUN水平无显著变化(P>0.05)。治疗后,各组UAER和血流变各指标较治疗前明显下降(P<0.01),联合治疗组下降幅度明显大于银杏叶组与舒洛地特组(P<0.05)。结论:银杏叶提取物联合舒洛地特治疗DNⅢ期患者能显著降低UAER和血液流变学指标,疗效优于单用银杏叶提取物或舒洛地特。Objective:To observe the clinical effect of Ginkgo extract combined with Sulodexide in diabetic nephroathyⅢpatients.Method:One hundred and twenty-eight patients with early of diabetic nephroathy were randomly divided into four groups:Ginkgo extract combined with Sulodexide treated group(combined treatment group),Ginkgo extract treated group,Sulodexide treated group and control group.Combined treatment group:to give Ginkgo extract 2pieces three times a day and Sulodexide250LSU twice a day.Ginkgo extract treated group:only to give Ginkgo extract 2pieces three times a day.Sulodexide treated group:only to give Sulodexide 250LSU twice a day.Control group:only to give symptomatic treatment.All groups were observed for eight weeks.The level of Scr,BUN,UAER,whole blood viscosity,plasma viscosity,plasma fibrinogen,erythrocyte maximum gathered rate and hematocrit were measured before and after treatment.Results:Compared with other three groups,in combined treatment group the level of UAER,whole blood viscosity,plasma viscosity,plasma fibrinogen,erythrocyte maximum gathered rate and hematocrit were significantly reduced after the eight weeks(P<0.05).Ginkgo extract treated group and Sulodexide treated group,there was no significant difference between them(P>0.05).Conclusion:Ginkgo extract combined with Sulodexide which were used in diabetic nephroathy Ⅲ patients were efficient,in particular can reduce the level of UAER,whole blood viscosity,plasma viscosity,plasma fibrinogen,erythrocyte maximum gathered rate and hematocrit.It was better than single-use Sulodexide or Ginkgo extract.
