腹腔镜联合小切口治疗120例老年泌尿系结石

摘　　要：目的分析腹腔镜联合小切口治疗老年泌尿系结石的临床疗效。方法选择本院泌尿外科于2008年7月至2012年7月间住院收治的120例老年泌尿系结石患者作为研究对象，年龄（64～78）岁，年龄平均（69．52±5．98）岁，筛选对象均无手术禁忌症，根据治疗方式不同分为使用常规开放性取石术的对照组及使用腹腔镜联合小切口治疗的观察组患者各60例，比较两组患者的治疗效果及手术相关指标的差异。结果观察组与对照组一次性取石成功率比较，差异无显著性统计学意义（P〉0．05）；观察组患者手术总时间（56．25±23．56）min、出血量（74．46±6．36）mL、术后并发症（8．33％）、使用止痛药（25％）观察患者引流量（28．67±3．32）mL、排气时间（16．42±5．62）h、下床活动时间（2．57±0．43）d均明显少于对照组患者（P〈O．05）。结论腹腔镜联合小切口可以有效提高老年泌尿系结石患者的治疗效果，缩短手术时间，减少手术出血，加速患者术后康复。Objective To analyze the clinical efficacy of laparoscopy combined with small incision in the treatment of eld- erly patients with urinary calculi. Methods The clinical data of 120 cases of senile urinary calculi were retrospectively ana- lyzed,including 60 patients treated with conventional open surgery as the control group,and 60 patients treated with laparosco- py in combination with small incision as the observation group. The efficacy and related indexes of the two therapeutic approa- ches were compared. Results There was no significant difference in the one-time success rate of stone removal between the two groups （P〉0.05）. The indexes of the observation group,including operation time （56.25 ± 23.56） min, amount of bleed- ing （74.4 ± 66.36） mL, postoperative complications （ 8.33% ）, the use of analgesics （ 25% ） observed in patients with drainage （28.67 ± 3.32） mL, exhaust time （ 16.42 ± 5.62 ） h, time to ambulation （ 2.57 ± 0.43） d, were significantly reduced compared with those of the control group （P〈0.05）. Oonclusion Laparoscopy combined with small incision can effectively improve the therapeutic effect of senile patients with urinary calculi, shorten the operation time, decrease operation hemorrhage, and promote recovery of patients after operation.
