自拟中药方湿热敷对湿热下注型混合痔术后消肿止痛作用的临床观察

摘　　要：目的观察自拟中药方湿热敷对湿热下注型混合痔(内痔Ⅲ期)术后的消肿止痛作用。方法将88例湿热下注型混合痔(内痔Ⅲ期)术后患者随机分为治疗组和对照组,每组44例。治疗组于术后用自拟中药方湿热敷,对照组用痔疾洗液湿热敷。两组疗程均为14天,分别于术后第2天、第7天、第14天观察出血、疼痛、渗液、充血及外痔水肿情况,并于疗程结束后评价临床疗效。结果①治疗组、对照组总有效率均为100%,组间临床疗效差异有统计学意义(P<0.05)。②治疗组术后第7天与第2天比较,渗液、充血分级积分差异有统计学意义(P<0.05);第14天与第7天比较,出血、疼痛、渗液、充血分级积分差异有统计学意义(P<0.05)。对照组术后第7天与第2天比较,充血分级积分差异有统计学意义(P<0.05);第14天与第7天比较,出血、疼痛、渗液、充血分级积分差异有统计学意义(P<0.05)。组间比较,第7天、第14天疼痛、渗液、充血分级积分差异有统计学意义(P<0.05)。③治疗组、对照组术后第7天与第2天、第14天与第7天、第14天与第2天比较,水肿面积差异均有统计学意义(P<0.05)。结论自拟中药方可明显减轻湿热下注型混合痔(内痔Ⅲ期)术后患者的肛门出血、疼痛、渗液、充血、水肿等术后症状,临床疗效优于痔疾洗液。Objective To observe the clinical effieacy of self-made prescription＇ s detumeseenee and analgesia eftbets for the treatment of postnperative mixed hereon-hold. Methods Eighty-eight subjects with postoperative mixed hemorrhoids of downward flow of damp-heat pattern were randomized intn treatment group and control group, with 44 subjects each group. The subjects in treatment group were treated with self-made prescription, and subjects in control group were treated with lotion by hot compress, with the course of 14 days. We observe the symptoms of bleeding, pain, hyperaemia and edema at the 2nd, 7th, 14th day after operation, and the clinical efficacy was evaluated after treatment. Results [1] The total effective rate was 100% in the treatment group and control group, with a differenee hetwcen the two groups （P 〈 0.05 ）. [2] At the 2nd and 7th day alter operation in treatment group, there were significant differences in the scores of effusion and bleeding （P 〈 0.05 ） ; at the 14th day and 7th day after operation, there were significant differcnees in the senres of bleeding, pain, etlhsinn and hyperaemia （ P 〈 0.05 ）. In eontrol group, there was signifieant dift）rcnee in hyperacmia seale between the 7th day and 2nd day after operation （ P 〈 0.05 ） ; and there were significant differenees in seores of bleeding, pain, effusion and hyperaemia between the 14th day and 7th day after operation （P 〈0.05 ）. At the 7th and 14th day, there were significant differenees in seores of pain, effusion and hyperaemia between the two groups （ P 〈 0.05 ）. [3] Compared the 7th day with 2nd day, the 14th day with 7th clay, the 14th clay and 2nd clay, no significant difference was found in area of edema alleviating either in treatment group or control group （ P 〈 0.05 ）. Conclusion The self-made prescription can alleviate the symptoms of bleeding, pain, effusion, hyperaemia, edema after operation in patients with mixed hemorrhoid of downward flow of damp-heat pattern, hs theraputie effect is
