摘  要:目的 :研究新生儿窒息后肺循环血流动力学的变化规律 ,提供早期无创的检测方法。材料与方法 :应用Ultra mark - 9型彩色超声诊断仪 ,对窒息新生儿肺循环血流量和肺动脉压力的变化进行了有对照的前瞻性动态观察。结果 :①新生儿窒息后肺循环血流量减少 ,肺动脉压力与阻力升高 ,严重者可导致持续胎儿循环 ;②窒息新生儿肺动脉压力的变化至生后一周末可恢复正常 ;③低氧血症与酸中毒是新生儿窒息肺循环血流动力学紊乱的病理学基础 ,且低氧血症与酸中毒的程度与肺动脉压力升高的程度具有高度相关性 (r=- 0 92 ,- 0 89,P <0 0 1)。结论 :肺循环血流动力学紊乱是新生儿窒息的重要病理生理变化 ,超声心动图可用于新生儿窒息后肺动脉压力变化的早期诊断和动态观察。Purpose:To study the variation of hemodynamics of pulmonary circulation in neonates with birth asphyxia and offer early diagnostic methods.Materials and Methods:The author prospectively and dynamicly observed the variation of pulmonary blood flow and pulmonary arterial pressure in asphyxiated neonates and healthy controls by using Ultramark-9 echocardiography.Results:①The pulmonary blood flow was decreased,the pulmonary arterial diastolic pressure (PADP),pulmonary arterial resistance(PAR) and pulmonary/systemic resistance ratios (PAR/RS) were elevated significantly(P<0 01).②The variation of pulmonary arterial pressure in asphyxiated neonates can be returned to normal at the end of one week after birth.③The degree of pulmonary hypertension negatively correlated with the degree of hypoxemia and acidosis (r=-0 92、-0 89,P<0 01).Conclusion:The hemodynamics disturbance of pulmonary circulation was an important pathophysiologic process in neonatal asphyxia,and the echocardiography can be used for the early diagnoses and dynamic observation of pulmonary arterial pressure in asphyxiated neonates.
