摘　　要：目的对石门县艾滋病防治效果进行卫生经济学评价,为合理配置卫生资源提供参考。方法用卫生经济学方法,结合国内相关研究资料,对2003-2012年石门县艾滋病防治效果进行评估。结果石门县艾滋病项目投入资金462.9万元,国家与县级财政投入之比为2.86:1,近5年的投入低于工作需求,投入产出的间接效益(CBA间接)为1:4.60,投入产出的直接效益(CBA直接)为1:11.24,项目成本效用(CUA)为847.1元/1人年,即挽救1个DALY为847.1元,项目避免264人感染HIV,避免了母婴感染,挽救病人的平均生存年为3.4年。结论以项目管理方式开展艾滋病防治工作,是一种全新的疾病控制模式,它将艾滋病工作分开目标化,整合资源进行有效管理,提高了工作效益。石门县2003-2012年艾滋病防治工作进行卫生经济学评价表明,艾滋病防治投入产生了明显的社会与经济效益。Objective To conduct an health economic assessment of HIV/AIDS prevention and treatment projects in Shimen County, and to provide references for reasonable allocation of health resources. Methods Health economic evaluation method was used to analyze the data on domestic related researches, and the effects of HIV/AIDS prevention and treatment projects in Shimen County during 2003 to 2012 were assessed. Results CNY 4. 629 million were invested in the AIDS projects in Shi- men County. The financial investment ratio of state to county was 2.86 ： 1. The input of recent 5 years was less than the work demands. The indirect cost - benefit analysis （CBA） of input and output was 1 ： 4.60, and the direct CBA of input and output was 1：11.24. The cost utility analysis （CUA） was 847.1 Yuan of a DALY, namely, it cost 847.1 Yuan to save a DALY. The projects prevented 264 individuals from infecting HIV, avoided HIV transmission from mother- to- child, and saved AIDS pa- tients with HARRT for the average survival of 3.4 years. Conclusions It is a new management mode of controlling the dis- ease that carries out the project of AIDS prevention and treatment. It aims at separating the targets and arranging the resources effectively so as to improve the work efficiency. Health economic evaluation on AIDS prevention and treatment in Shimen County during 2003 to 2012 indicates that the input of AIDS prevention and treatment has produced obvious social and economic benefits.
