大规模疾病筛查中胸部屏-胶组合和数字X线摄影质量分析

摘　　要：目的探讨提高大规模疾病筛查中胸部X线摄影质量的方法。方法对绵阳市建设重大传染病综合防治示范区项目中2010—2011年摄取的4235例胸部正位X线影像进行对比分析。结果 4235例胸部影像中屏-胶组合摄影3132例,数字X线摄影(digital radiography,DR)1103例。屏-胶组合摄影优片率为37.0%;DR摄影优片率为48.0%,DR摄影优片率明显高于屏-胶组合摄影优片率,差异有统计学意义(P<0.05);屏-胶组合摄影废片率6.8%,DR摄影废片率0.91%,DR摄影废片率明显低于屏-胶组合摄影废片率,差异有统计学意义(P<0.05)。223例废片中,体位因素、光学因素、伪影因素、标识因素引起的分别占17.0%、24.7%、44.4%、13.9%。结论在大规模疾病筛查中,与屏-胶组合摄影相比胸部DR摄影具有明显优势,但只有加强全程质量管理与控制,才能不断提高大规模疾病筛查中胸部摄影质量。Objective To discuss the methods as to improve the quality of screen-plastic chest X-ray radiography in large-scale disease screening. Methods Chest PA X-ray images of 4235 cases in a project for prevention and control of se- vere infectious diseases from 2010 to 2011 in Mianyang city were compared and analyzed. Results In the 4235 cases, there were 3132 cases of screen-plastic chest X-ray and 1103 cases of digital radiography （DR）. The excellent rate was 37.0% in screen-plastic chest X-ray radiography and 48.0% in DR photography respectively, and the rate in DR photography was signif- icantly higher than that in screen-plastic chest X-ray radiography （P 〈 0.05 ） ; the scrapped rate was 6.8% in screen-plastic chest X-ray radiography and 0.91% in DR photography respectively, and the rate in DR photography was significantly lower than that in screen-plastic chest X-ray radiography （ P 〈 0.05 ）. In 223 scrapped fihns, the ratios caused by factors of pos- ture, optics, artifacts and marking were 17.0% , 24.7% , 44.4% and 13.9%. Conclusion The effect by DR photography is much better than that by screen-plastic chest X-ray radiography in large-scale disease screening, and the quality can be im- proved by enhancing and total quality control and management.
