摘 要:目的探讨影响社区卫生服务机构医务人员工作积极性的因素。方法根据2012年卫生部卫生发展研究中心收集的各地呈报的基层医疗与公共卫生事业单位实施绩效工资的调查数据,抽取402家社区卫生服务机构。采用单因素及多因素非条件Logistic回归分析方法分析影响医务人员工作积极性的因素。结果实施绩效工资后16.7%(67/402)的社区卫生服务机构医务人员的工作积极性明显提高,65.4%(263/402)提高,17.9%(72/402)没有提高;单因素非条件Logistic回归分析结果显示,机构是否创造良好工作环境、提供良好职业发展、制定合理工资水平、提供培训机会、合理确定编制、职务晋升机会、承担重要任务、享受休假休息进入回归模型;多因素非条件Logistic回归模型筛选出影响医务人员工作积极性的主要因素是合理工资水平。结论 82.1%的社区卫生服务机构实施绩效工资后医务人员积极性有提高,合理的工资水平是影响社区卫生服务机构医务人员积极性的关键因素。Objective To analyze influencing factors for working enthusiasm of health workers in community health service facilities. Methods By stratified -cluster random sampling, 402 community health service facilities were selected from the country's 3t provinces or municipalities and Xinjiang Production and Construction Corps (excluding Tibet) . Questionnaire surveys were conducted to collect data on workers performance related payment system and the facilities. Influencing factors of working enthusiasm of health workers were analyzed through single factor and multiple factors non - conditional Logistic regression method. Results After implementing performance related payment system, 16. 7% (67/402) of health workers' enthusiasm of community health service facilities were obviously improved, 65.4% (263/402) were improved, and 17.9% (72/402) were not improved; Single factor Logistic regression model showed that factors influencing workers' working enthusiasm including whether the facility could provide favorable working environment, career development, reasonable payment system and compilation, training opportunity, promotion, key tasks and vacation. Multi -factor Logistic regression model showed that the factors influencing health workers' enthusiasm were mainly the reasonable payment level. Conclusion 82. 1% of health workers' enthusiasm of community health service facilities was improved after implementing performance related payment system. Reasonable payment level and rational distribution of income among health workers and leaders are the key factors influencing health workers' en- thusiasm in the community health service facilities.
