使用气囊导尿管致男性尿道损伤分析

摘　　要：目的　探讨因留置导尿管所致的男性尿道损伤的原因和治疗方法。方法　对 13例尿道损伤的临床资料进行分析。结果　全部为住院患者 ,年龄 2 3岁～ 79岁 ,其中内科 5例 ,外科 8例。因导尿而损伤 10例 ,拔尿管损伤 3例。尿道部分裂伤 10例 ,尿道粘膜损伤 3例。其中前尿道 6例 ,后尿道 4例 ,全尿道 3例。除 1例未置尿管外其余 12例均重新置导尿管且全部成功 ,拔管后排尿正常 ,无并发症。结论　气囊导尿管致尿道损伤多由于操作不当引起 ,多为尿道部分裂伤和粘膜损伤 ,一旦发现应及时处理 ,重新插导尿管困难者 ,先用尿道扩张器探入膀胱 ,再用自制的钢丝芯引导尿管入膀胱 ,保留 10～ 14d,疗效满意。Objective To explore the reason and treatment method resulting from using catheter injured male urethra.Methodsanalyzing 13 cases injured urethra clinical material.Results all of them were in patients,age ranked from 23～79years old,therein medical department 5 cases,surgical department 8 cases.injury resulted from leading urine 10 cases,injury resulted from pulling out catheter 3 cases.urethra injury 10 cases,urethramembrane injury 3 cases.therein front urethra 6 cases,behing urethra 4 cases,full urethra 3 cases,all of 12 cases anew installed catheter except for 1 cases and were successful comletely,emiction was normal after put out tube,and without other disease.Conclusions because of operating improper,air cell catheter injured uretha,most of them were urethra injury and mucous membrane injury,once fou yt,should deal with it in time,if it was diffcult to reinsert catheter,first put dilater into bladder,then led cather into bladder with selr made steel wrire core,kept it o 10～14 days,ourative effect was satisfactory.
