野战条件下锁骨骨折新式手术方式的研究

摘　　要：目的 研究野战条件下治疗锁骨骨折的新术式.方法 2008年1月-2010年7月,锁骨骨折22例采用微创空心螺钉髓内固定术（新术式）.术后采用Neer肩关节活动评分对患肩进行评价.摄锁骨正位X线片观察骨折愈合情况,并与同期采用钢板固定组的53例进行对照.结果 所有患者均获得随访至骨折愈合,随访时间4-8个月.空心螺钉组Neer评分为97分.X线片证实全部病例骨折愈合,愈合时间3-5个月,平均3.5个月.无空心螺钉松动、弯曲、断裂及骨折错位等.与钢板组固定组比较,手术时间、术中出血量、Neer肩关节功能评分无统计学意义（P＞0.05）.结论 野战条件下,采用空心螺钉治疗锁骨骨折的术式具有效果确切,手术时间短,器械简单轻便价廉.如能得到普及和推广,必将有利于提高战场卫勤救治效率.Objective Study on new treatment of clavicle fracture with hollow screw in field battle. Methods From January 2008 to July 2010,22 patients with clavicle fracture were treated with hollow screw. The anterior median incision of 4 -5 cm was made to explore the clavicle fracture. Use a guide pin through the Pulp cavity point to distal aeromion, and return back to fracture proximal, and hollow screw was implant to fix clavicle. All patients were physically examined by Neer score. Fracture in all patients was evaluated by clinical examination and radiography. There was another group of 53 patients treated with plate. Results All cases were followed up postoperatively for 4 - 8 months. Neer score of hollow screw group was 97 points, and the cases were proved be bone healing by clinical examination radiology with a mean of 3.5 months （ range 3 - 5 ）. There was no screw broken and fracture displacement. There were significantly difference between the two groups （P 〉 0. 05 ）. Conclusion New treatment of clavicle fracture with hollow screw in field battle is suggested. It is easy to be carried, and took less time to operate. It has a good practice value in field battle.
