肠道病毒71感染所致手足口病患儿Th17细胞的变化及临床研究

摘　　要：目的研究肠道病毒71（EV71）感染所致手足口病患儿体内Thl7细胞及相关因子IL一17表达的变化，以明确其在小儿手足口病发生发展中的意义。方法选择2012年6月一2013年1月EV71感染的手足口病住院患儿69例，年龄6月龄～7岁，将其分为重症组26例，普通组43例，另选32名健康幼儿作为对照组，采用酶联免疫法检测血清中IL一17和11．-23水平；采用流式细胞术检测外周血中Thl7及Thl、Th2细胞。结果普通组和重症组患儿血清中IL一17水平分别为（101．25±14．62）pg／ml和（139．34±18．91）pg／ml，IL一23水平分别为（72．38±11．85）pg／ml和（108．43±15．37）pg／ml，对照组IL-17水平（28．41±5．33）pg／ml、IL-23水平（31．67±8．69）pg／ml，普通组和重症组与对照组差异有统计学意义（P〈O．05）；普通组和重症组患儿体内的Thl7细胞表达显著高于对照组的（O．19±0．04）％，分别为（O．56±0．21）％和（O．98±0．35）％，差异有统计学意义（P〈O．05），Thl细胞在普通组（2．75±0．97）％和重症组（1．69±0．82）％患儿的表达显著低于对照组（4．16±1．23）％，而Th2细胞则与Thl7细胞在各组有相似的变化趋势。结论Thl7细胞及其分泌的IL一17参与了EV71感染所致手足口病的发生与进展，对患儿病情的严重程度和病情的转归具有提示意义。OBJECTIVE To investigate the change of Th17 cells expression and related eytokine IL-17 in children who suffered from hand-foot-mouth disease （HFMD） infected by human enterovirus 71 （EV71）, so as to understand the meaning of it for the incidence and development of HFMD. METHODS A total of 69 HFMD children inpatients with EV71 infection from Jun. 2012 to Jan. 2013~ age from 6 months to 7 years old were chosen; the patients were divided into severe group （26 cases）, mild group （43 cases） ~ control group was added with other 32 healthy children. ELISA was used to detect serum IL-17 and IL-23~ FACS was used to detect Thl7, Thl and Th2 cells in peripheral blood. RESULTS The levels of IL-17 in the mild group and severe group were （101. 254-14.62）pg/ml and （139.34_--t-18.91）pg/ml~ the levels of IL-23 were（72.38_＋11.85） pg/ml and （108.43 ~15.37） pg/ml in the two groups; the level of IL-17 and IL-23 were （28.41 t5.33）pg/ml and （31.67_＋8.69） pg/ml in control group,and there were significantly difference between the comparison of control group with mild and severe groups （P〈0. 05）. Compared with control group （0. 19~0.04）~, expression of Th17 cells were significantly increased in mild group （0.56~0.21）~ and severe group （0. 98 ~ 0.35）~, and the difference has statistical significance（P〈0. 05）. Compared with control group（4.16_--t-1.23）%, the Thl cell erpression were significantly lower in mild group（2.75-t-0.97） ~ and severe group （1.69_--＋-0.82） ~, while Th2 cell erpression had the similar change with Th17 in those groups. CONCLUSION Thl7 cells and IL-17 participate in EV71 infection induced HFMD, which has the recognizing meaning for the improvement of severe disease in children with HFMD.
