局部麻醉下无张力疝修补术治疗高龄腹股沟疝146例

摘　　要：目的观察局部麻醉下无张力疝修补术治疗高龄腹股沟疝的疗效。方法回顾性分析2010年4月至2012年5月,广西中医药大学附属瑞康医院对146例高龄腹股沟疝患者行无张力疝修补术。对手术时间、术后疼痛、并发症、复发率等项目进行分析。结果本组患者全部治愈,平均手术时间为42 min;平均住院时间4.1 d。术后发生尿潴留2例,阴囊积液2例,阴囊血肿1例,无切口感染。随访6个月至2年,均无复发。结论局部麻醉下行无张力疝修补术可显著减少高龄腹股沟疝复发率,手术创伤小、术后恢复快、并发症少、手术适应证相对拓宽,尤其适合于治疗高龄股沟疝患者和(或)伴有其他疾病者。Objective To explore the value of tension-free hernia repair under local anesthesia in the treatment of senile patients with inguinal hernia. Methods A total of 146 patients with inguinal hernia in Ruikang Hospital Affiliated to Guangxi University of Chinese Medicine from April 2010 to May 2012 were enrolled and retrospectively analyzed. The operation time, postoperative pain, complications and recurrence rate were assessed. Results All the patients were cured. The mean of operation time was 42 minutes and the mean of hospitalization stay lasted 4.1 days. There were 2 cases of urinary retention,2 cases of serotal hydroceles, 1 case of scrotal haematoma without wound infection. No recurrence was observed and followed up from 6 months to 2 years. Conclusions The tension-free hernia repair under local anesthesia could effectively reduce the recurrence rate with mini-invasive approach, rapid recovery, less complication and wide indications, especially for those senile patients with inguinal hernia and those complicated with other diseases.
