经会阴巴德补片植人治疗直肠前突的临床观察

摘　　要：目的探讨经会阴巴德补片植入治疗直肠前突的临床效果。方法选择2008年12月—2013年1月确诊的直肠前突患者56例,随机分为观察组和对照组各28例,观察组给予经会阴巴德补片植入治疗,对照组给予直肠闭式修补术。术后评价2组患者的临床疗效、症状评分,同时统计2组患者并发症发生情况。结果 2组患者治疗后1周、3个月症状评分均较治疗前明显减少(P<0.05,P<0.01),治疗后3个月观察组患者的症状评分明显低于对照组(P<0.05);观察组患者治愈率、总有效率(89.3%,96.4%)明显高于对照组(75.0%,85.7%)(P<0.05);对照组发生并发症7例(25.0%),其中性交痛5例,阴道狭窄2例,观察组仅出现性交痛2例(7.1%),2组比较差异有统计学意义(P<0.05)。结论经会阴巴德补片植入治疗直肠前突是一种有效的方法,并发症发生率低,远期效果满意,安全可靠,值得临床推广。Objective To explore the clinical effects of perineum Bard patch implantation for treatment of rectocele.Methods Select 56 patients who were diagnosed of rectocele from December 2008 to January 2013,they were randomly divided into observation group and control group with 28 cases in each group,observation group received transperineal Bard patch implantation therapy,the control group received the rectum closed repair.Postoperative evaluation of the clinical efficacy of the two group,symptom scores,and complications occur rate.Results After 1 week of treatment,and the first three months,compared within the 2 groups,therapy significantly reduced symptom scores in both groups（P 0.05,P 0.01）,after 3 months of treatment,the patient＇s symptom score in observation group was significantly lower than the control group（P0.05）;cure rate and the total efficiency（89.3%,96.4%）of patients in the observation group was significantly higher than control group（75.0%,85.7%,P 0.05）;complications occurred in seven cases in the control group（25.0%）,including 5 cases of dyspareunia,vaginal stenosis in 2 patients in the observation group appears only dyspareunia two cases（7.1%）,statistically significant difference was found between the two groups（P 0.05）.Conclusion Bard patch transperineal rectocele pre-implantation therapy is an effective way to lower complication rate,long-term results were satisfactory,safe and reliable,worthy of promotion.
