摘  要:目的探讨应用心脏超声筛查高危新生儿先天性心脏病(congenital heart disease,CHD)价值。方法 2007年1月—2012年6月新生儿病区接受心脏彩超检查的符合标准的高危新生儿782例,对CHD的发病情况及类型进行分析。结果 782例心脏彩超检查结果异常404例(51.66%),其中非青紫型CHD 350例(86.63%),青紫型CHD 54例(13.37%)。非青紫型CHD发病率居前四位的是动脉导管未闭(patent ductus arteriosus,PDA)、房间隔缺损(atrial septal defect,ASD)、室间隔缺损(ventricular septal defect,VSD)和PDA+ASD,以上4种类型占非青紫型CHD的83.43%;ASD以中央型居多,VSD以中小型缺损居多。青紫型CHD发病率居前4位的是大动脉转位(complete transposition of great arteries,TGA)、法洛四联症(tetralogy of fallot,TOF)、依赖动脉导管供应肺循环的CHD、完全型肺静脉异位引流(total abnormal pulmonary venous drainag,TAPVD)。本组青紫型CHD常合并多种心脏畸形。结论新生儿CHD类型众多,尽早应用彩色多普勒超声心动图对高危新生儿进行检查可明确诊断。Objective To explore the value of ultrasound screening for high-risk newborns with congenital heart disease (CHD). Methods Clinical data of 782 high-risk newborns undergoing echocardiography examinations during January 2007 and June 2012 was researched, and the incidence rate and types of CHD were analyzed. Results .In the 782 newborns, 404 newborns (51. 66 %) had abnormal cardiac ultrasound, in which there were 350 newborns (86.63%) with non-cyanotic congenital heart disease and 54 newborns (13.37%) with cyanotic congenital heart disease. The top four incidence rates in non-cyanotic congenital heart disease group were patent ductus arteriosus (PDA) ; atrial septal defect (ASD) , ventricular septal defect (VSD) and PDA + ASD, and the percentage of the 4 types of diseases to the non-cyanotic congenital heart disease was 83.43 % ; ASD was mostly of central defect, and VSD was mostly of small and medium-sized defects. The top four incidence rates in cyanotic congenital heart disease group were transposition of great arteries (TGA) ; tetralogy of Fallot (TOF) , congenital heart disease with duct-dependent pulmonary circulation and total anomalous pulmonary venous drainage (TAPVD). The cyanotic CHD in the group was often complicated with multiple cardiac malformations. Conclusion There are many different types of CHD. and suspicious high-risk newborns should undertake the color Doppler echocardiographic examination as quickly as possible to confirm the diagnosis.
