茵陈退黄方治疗新生儿高胆红素血症的临床研究

摘　　要：目的:通过茵陈退黄方治疗新生儿高胆红素血症的临床研究,评价其临床疗效,寻求最佳用药途径和剂型。方法:符合新生儿高胆红素血症的诊断标准,中医辨证属于湿热内蕴(阳黄)的180例患儿,按随机方法分为对照组、汤剂治疗组和栓剂治疗组3组。对照组采用常规治疗,汤剂治疗组口服茵陈退黄汤,栓剂治疗组使用茵陈退黄栓,3组均7d为一疗程。疗程结束后第2天,日龄为14d和28d时各随访1次。结果:茵陈退黄栓、茵陈退黄汤治疗新生儿高胆红素血症总有效率优于常规治疗(P<0.05);3组黄疸缓解率比较无统计学差异;对照组、汤剂治疗组、栓剂治疗组黄疸复发率分别为25.71%、7.69%和5.71%,茵陈退黄方治疗新生儿高胆红素血症的黄疸复发率较常规治疗低(P<0.05)。结论:茵陈退黄方治疗新生儿高胆红素血症总有效率高,且复发率低。Objective： To find the best route of administration and dosage forms, throught the clinical study on the clinical effect of resolving neonatal hyperbilirubinemia with Virgate Wormwood Herb Suppository （Soup）. Methods： We divided 180 cases of newborn who met the diagnostic criteria for neonatal hyperbilirubinemia and belong to the accumulation of damp-heat in the body based on syndrome differentiation of traditional Chinese medicine （yang jaundice） into the control group, the decoction treatment group and the suppository treatment group with random method. The control group with conventional treatment, the decoction treatment group with the Virgate Wormwood Herb Soup, the suppository treatment group with the Virgate Wormwood Herb Suppository, the courses of treatment of all the three groups were 7 days. We followed up the newborns in the 2nd day after the completion of the course, the age 14 and 28 days. Results： The total efficiencies with the Virgate Wormwood Herb Suppository and the Virgate Wormwood Herb Soup in treatment of neonatal hyperbilirubinemia were superior to conventional treatment （P〈0.05）. The remission rates of three groups in jaundice were no statistically significant. The each jaundice recurrence rate of three groups was 25.71%, 7.69% and 5.71%. The jaundice relapse rate with the Virgate Wormwood Herb Suppository （Soup） in the treatment of hyperbilirubinemia jaundiceare was lower than conventional treatment （P〈0.05）. Conclusion： The total efficiency with the Virgate Wormwood Herb SuppositOry （Soup） in treatment of neonatal hyperbilirubinemia is higher than the control group and the recurrence rate is lower.
