摘  要:目的探讨结合组织多普勒的Tei指数在评价胎儿先天性心脏病左心功能早期变化的价值。方法选择经常规产前超声检查发现胎儿心脏结构或心律有异常的孕妇27例,以30例胎儿心脏结构或心律正常者为对照组。在常规胎儿超声心动图切面测量胎儿左室舒张末期内径及收缩末期内径(LVDd、LVDs)、左室射血分数(LVEF%)、左室缩短率(FS%),应用组织多普勒于心尖五腔心切面测得Tei指数〔Tei指数=(ICT+IRT)/ET=(a-b)/b〕。对各组的EF、FS及Tei指数进行分析。结果对照组胎儿的LVDd、LVDs、EF、FS及Tei指数没有明显变化;先心组EF%、FS%轻度减低,但是没有明显统计学意义;与对照组相比,先心组LVDd、LVDs没有明显统计学意义,而Tei指数相对于对照组则明显增高(P<0.01)。结论结合组织多普勒的Tei指数能早期敏感检测胎儿早期心脏功能的变化,为评估胎儿先天性心脏病早期心功能改变提供了客观依据。Objective To discuss the value of early cardiac functional change for ictus with congenital heart disease by using Tel index combine TDI. Methods 27 Gravidas whose fetus proved to be abnormal in cardiac structure or arrhythmia through ultrasound were employed as study group ( n = 27 ), another 30 gravidas whose fetus suffered none congenital heart disease were collected as control group. Measure the LVDd, LVDs, LVEF% and FS% under ultrasound cardiogram, and got Tei index ( Tei index = ( ICT + IRT)/ET = ( a - b)/b ) under TDI at five tune section of heart apex. Analyze EF, FS and Tei index of each group. Results LVDd, LVDs, EF% , FS% and Tei index had no obvious change in control group. Compared to control group EF% , FS% decreased slightly, but present no statistically significant, so did LVDd, LVDs, however, Tei index of study group increased significantly compared to control group( P 〈 O. O1 ). Conclusions Tei index combine with TDI could be a sensitively method for detect early cardiac functional change for fetus suffered congenital heart disease, and provide evidence to assess early cardiac functional change for fetus with congenital heart disease.
