B超引导下膀胱穿刺置管治疗有下腹部手术史尿潴留患者的临床探讨

摘　　要：目的总结B超引导下膀胱穿刺留置中心静脉导管治疗有下腹部手术史的前列腺增生症并尿潴留患者的经验。方法报告5例下腹部手术史尿潴留病例资料并复习相关文献。5例均为男性,年龄59~89岁,其中2例10余年前行耻骨上前列腺摘除术,1例于5年前行膀胱切开取石术,1例于5年前行直肠癌根治术,1例于3年前行膀胱部分切除术。5例患者均留置导尿管失败,行B超引导下膀胱穿刺留置中心静脉导管。结果 5例均一次性穿刺成功并顺利留置中心静脉导管,术后引流通畅,无肉眼血尿,无导管脱出等。结论 B超引导下中心静脉导管膀胱穿刺造瘘治疗有下腹部手术史尿潴留患者是一种安全、有效的治疗方法。【Objective】To summarize the experience of suprapubic puncture cystostomy under the B-ultrasound guiding in urinary retention patients with history of abdominal operation.【Methods】All 5 cases were male patients,59 to 89years old. 2 cases of them underwent suprapubic prostatectomy,1 case with a history of bladder lithotomy,1 case underwent rectal cancer radical surgery, 1 case received partial cystectomy. All patients failed to indwell catheters, and underwent suprapubic puncture cystostomy under the B-ultrasound guiding to indwell central venous catheter. The relevant literatures were reviewed.【Resluts】5 cases were indwelled central venous catheters successfully by the first puncture,drainage unobstructed postoperative, no gross hematuria or catheter prolapse.【Conclusion】In urinary retention patients with history of abdominal operation, suprapubic puncture cystostomy under the B-ultrasound guiding to indwell central venous catheter is safe and effective.
