糖尿病大鼠足突间隙宽度与尿nephrin的相关性及益气养阴活血通络方的影响

摘　　要：目的观察实验性糖尿病(DM)大鼠足细胞足突间隙宽度(SD)与尿nephrin(U-nephrin)的相关性及益气养阴活血通络方的影响。方法除10只正常对照组大鼠外,高脂饲料饲养联合30 mg/kg STZ腹腔注射复制DM大鼠模型,72 h后空腹血糖≥16.7 mmol/L的成模大鼠随机分为模型组和益气养阴活血通络方(由中药配方颗粒黄芪、地黄、丹参、川芎各2袋,黄精、地龙、水蛭、全蝎各1袋组成)治疗组各10只。治疗组按1.00 g/kg定容至灌胃量1ml/100g日1次给药,正常对照组和模型组按相应体积凉开水灌胃,32周后检测糖化血红蛋白(HbA1c)、尿白蛋白排泄率(UAER)、U-nephrin、SD,分析模型组U-nephrin与SD之间的关系。结果与正常对照组比较,模型组UAER、HbA1c、U-nephrin显著升高(P<0.05),SD显著增宽(P<0.05),U-nephrin与SD呈正相关,相关系数0.941。与模型组比较,治疗组HbA1c、UAER、U-nephrin显著下降、SD增宽显著改善(P<0.05)。结论益气养阴活血通络方可显著减少尿蛋白,对糖尿病肾病有积极治疗作用,可能与其减少U-nephrin、抑制SD增宽从而减少尿蛋白排泄有关。Objective To investigate the relation between width of podocytic process slit diaphragm gap（SD） and urinary nephrin （ U - nephrin） of experimental diabetic rats and effects of yiqi - yangyin - huoxue - tongluo recipe. Methods Except 10 rats of normal group（NG） ,experimental DM model was induced by high fat diet combined with 30mg/kg STZ intraperitoneal injection. The diabetic rats whose fasting blood sugar〉 16.7mmol/L after 72 -hours were randomly divided into model group（MG） and yiqi -yangyin- huoxue- tongluo recipe（ which included decoction- free herbal granule of two packages of Huangqi, Dihuang, Dansh- en, and Chuanxiong, one package of Huangjing, Dilong, Shuizhi and Quanxie） treatment group （YHTG） , 10 rats respectively. The recipe was administered intragastrically once dally by dosage of 1.00g/kg dissolved into lml/100g for 32 weeks,while rats of NG and MG gavaged cool boiled water with corresponding volume. At the endpoint of the study, HbA1 c, urinary albumin excretion rate （UAER） ,urinary nephrin （ U - nephrin） ,and SD were measured. The relation between U - nephrin and SD of MG was analysed. Results Compared with the NG, HbA1 c, UAER, U - nephrin of MG increased notably （ P 〈 0.05 ）, and SD broadened markedly （ P 〈 0.05 ）. U - nephrin and SD had a positive correlation, correlation coefficient was 0. 941. Compared with MG, HbA1 c, UAER, U - nephrin, and SD of YHTG were improved markedly （ P 〈 0.05 ）. Conclusion Yiqi - yangyin - huoxue - tongluo recipe can notably reduce urinary albumin, and shows a positive and therapeutical effect on diabetic nephropathy, which may be related with preventing the loss of nephrin in urine and inhibition of SD broadening so decreasing urinary albumin excretion.
