唐山市开平区2008-2010年居民主要死因潜在减寿年数分析

摘　　要：目的通过对唐山市开平区2008-2010年死亡人群的调查研究,了解该地区死因顺位及死因构成情况,分析居民主要死因的疾病负担。方法使用Excel2003等软件计算和分析居民主要死因的死亡率、构成比、潜在寿命损失年数等指标。结果 2008-2010年开平区累计死亡4 844人,平均死亡率为657.79/10万,其中,男性平均死亡率为726.91/10万,女性平均死亡率为589.16/10万。死因顺位前五位依次为:脑血管病、心脏病、恶性肿瘤、伤害、呼吸系统疾病。居民全死因PYLL为43 019.50人年,AYLL为8.88年;累计WPYLL为21 637.50人年,AWYLL为4.47年。结论针对心脑血管疾病、恶性肿瘤及伤害死亡率较高等问题,采取相应的预防措施。此外还应通过加大政府资金投入,建立、健全网络直报规范制度等途径提高网络直报质量,切实提高直报系统数据的可利用性。Objective The aim of this study was to figure out the composition and the rank of mortality cause in Kaiping district Tangshan city based on the investigation of mortalities from 2008 to 2010, and to analyze the disease burden of the main mortality causes for the residents. Methods Excel 2003 was used to calculate and analyze the indexes such as mortality rate, constituent ratio for the major mortality causes, and the potential years of life lost （PYLL） among the residents. Results The mortality toll in this area was 4 844 from 2008 to 2010, with an average mortality rate of 657.79/100 000. The average mortality rate for male was 726.91/100 000 and the mortality rate for female was 589.16/100 000. The top five causes for mortality were cerebrovaseular disease, heart disease, malignant tumors, accidental injuries and respiratory diseases. The PYLL for mortality causes in the whole population was 43 019.50 person- years, while the average years of life lost （AYLL） was 8.88 years. The accumulated WPYLL was 21 637.50 person-years, and the average working potential years of life lost （AWPYLL） was 4.47 years. Conclusion Due to the high mortality rates for cardiovascular and cerebrovaseular diseases, malignant tumors and accidental injuries, corresponding preventive strategies should be taken. In addition, the quality of direct reporting network system should be improved by increasing government funding and establishing/perfecting regulation system, in order to increase data availability and maximize corresponding effects.
