摘  要:目的通过比较不同授精浓度下胚胎发育及妊娠结局,探讨体外受精-胚胎移植(WF-ET)中较为合适的授精浓度。方法对因输卵管因素接受1VF-ET的夫妇进行随机对照试验,根据不同授精浓度随机分为3组:A组为(0.1~0.5)×106/mL,B组为(0.5-1.0)×10^6/mL,C组为(1.0-1.5)×106/mL。比较3组间正常受精率、多精受精率、卵裂率、优质胚胎率、临床妊娠率、着床率及流产率。结果3组的正常受精率、卵裂率、优质胚胎率及流产率均无统计学差异(P〉0.05),但B组和C组的多精受精率(分别为14.9%和13.2%)明显高于A组(8.6%)(P〈0.05),而且C组的妊娠率及种植率(50.0%,29.5%)明显高于B组(41.4%,20.4%)和A组(38.6%-18.8%)(P〈0.05)。结论IVF.ET治疗中,增加授精滴中精子浓度可导致卵子多精受精增加,但在一定范围内增加精子浓度可能改善妊娠结局。Objective To explore the optimal sperm insemination concentration for the in vitro fertilization-embryo transfer (IVF-ET) procedure through comparison of the outcomes of IVF-ET with different sperm insemination concentrations. Methods A randomized controlled trial was car- fled out in 210 couples who received IVF-ET treatment for tubal factor infertility. Couples were randomly divided into three groups according to the insemination concentrations: group A with concentration (0.1-0.5) x 106/mL, group B (0.5-1.0) x 106/mL, and group C ( 1.0-1.5 ) x 106/mL. Normal fertilization rate, polyspermy rate, cleavage rate, high-quality embryo rate, pregnancy rate, implantation rate and abortion rate were compared be- tween the three groups. Results No significant difference was found in normal fertilization rate, cleavage rate, high-quality embryo rate and abortion rate between the three groups (P 〉 0.05). However, the polyspermy rates of group B ( 14.9% ) and group C ( 13.2% ) were significantly higher than that of group A ( 8.6% ) (P 〈 0.05 ). In addition, the pregnancy rate ( 50.0% ) and implantation rate (29.5%) of group C were significantly higher than those of group B (41.4% and 20.4% ) and group A ( 38.6% and 18.8% ), respectively (P 〈 0.05 ). Conclusion Increasing the sperm concen- tration of insemination in the ctdture droplet in IVF-ET treatment may increase the risk of polyspermy fertilization. However, increasing the sperm in- semination concentration within a certain scope may improve the treatment outcome.
