摘  要:目的观察重建糖尿病下肢血管对血管内皮生长因子及内皮素的影响及其临床疗效。方法应用微球囊扩张加脐血干细胞、微球囊扩张、脐血干细胞移植3种方法治疗糖尿病下肢缺血性血管病变,观察治疗前后局部皮肤温度、踝肱指数(ABI)、经皮氧分压、血管内皮生长因子(VEGF)、内皮素(ET-1)变化。结果微球囊扩张加干细胞组治疗后各观察指标的改善明显优于其他两组,特别是VEGF和ET-1,6个月后开始出现明显改善(P<0.05),12个月后改善更加显著(P<0.01);血管造影示干细胞治疗区域毛细血管网明显增多。结论 3种方法治疗糖尿病下肢血管病变均有效。微球囊扩张加脐血干细胞移植组治疗糖尿病下肢血管病变具有较好的近期及远期效果。VEGF和ET-1可作为糖尿病下肢缺血性血管病变改善有效性的评价指标。Objective To observe the effect of vascular reconstruction on the indexes of VEGF and ET-1 in the treat- ment of diabetic lower extremity vascular disease. Methods Micro balloon dilatation combined with umbilical cord blood stem cells,micro balloon dilatation and umbilical cord blood stem cell transplantation were applied to treat the diabetic lower extremi- ty vascular disease. Observed the changes of local skin temperature, ABI,transcutaneous oxygen pressure ,VEGF and ET-1 be- fore and after treatment. Results The indexes above observed,particularly,VEGF and ET-I in the combined treatment group were much obviously improved than that in the other control groups after 6 months of treatment (P〈0.05) and after 12 months of treatment (P〈0.01). The capillary network in the stem cell treatment regional significantly increased under angiography. Con- elusion Three methods in the treatment of diabetic lower extremity vascular disease are effective. The micro balloon dilatation combined with umbilical cord blood stem cells treatment can obtain good short-term and long-term effect. VEGF and ET-1 can be used as effective indexes to evaluate the improvement of diabetic lower extremity vascular disease.
