血液透析滤过对急性肾衰竭患者超敏C反应蛋白水平的影响

摘　　要：目的探讨急性肾衰竭（acute rena lfailure，ARF）超敏C反应蛋白（high sensitivity C reactive protein，hs-CRP）水平变化及其与预后的关系。方法回顾性分析97例临床符合ARF诊断标准的患者，收集其病史、治疗经过及治疗过程中所检测的hs-CRP数值；同时取健康的52例患者，检测其hs-CRP数值作为对照。所有入组患者均给予常规血液透析滤过（hemodiafihration，HDF）治疗。结果①老年组治愈3例，好转6例，未愈17例，死亡25例。非老年组治愈8例，好转11例，未愈21例，死亡6例，2组比较差异有统计学意义（P〈0．05）；②Hs-CRP检测值比较，正常健康人群组hs-CRP平均值为（5．1±2．76）mg／L，治疗前老年组、非老年组与正常健康人群组比较，差异有统计学意义（P〈0．05）。老年组与非老年组治疗前、后比较，差异有统计学意义（P〈0．05）；③多因素Lo-gistic回归分析，性别（男）、年龄（老年）、高水平hs-CRP是预后不良的危险因素，一周内HDF为保护性因素。结论高水平hs-CRP检测值为影响ARF患者预后的因素之一。有效降低hs-CRP检测值，减轻机体的炎症反应能增加ARF患者治愈率。Objective To （hs-CRP） protein in patients of study prognosis and changes in the acute renal failure（ARF）. Methods levels of high sensitivity C reactive A total of 96 patients in accord- anee with the diagnostic criteria of ARF were analyzed retrospectively, the history, treatment and de- tection of the hs-CRP values in the process of treatment were collected, the detection of the hs-CRP values in comparison were taken in 52 healthy patients at the same time. All the groups o{ patients were given the hemodiafiltration（HDF） treatment. Results （l）In the elderly group, 3 cases were cured,6 cases were improved and 17 cases ineffective,25 cases of death. In non-elderly group, 8 cases were cured, l I cases improved and 2l cases ineffective, 6 cases of death,between two groups there was statistical significance （P^0. I）5）. （2）In the healthy patients average of hs-CRP is （5.1 ＋ 2. 76） mg/L,bdore the treatment, compared with the heahhly patient group, the elderly and non-elderly group was statistically significance （P^0. （）5）;Compared with before treatment, the elderly and non- elderly group was statistically significance （P^0. I）5） after treatment. （3）After multiariable Logistic regression analysis, the gender （male）, the age（older） and the high levels of hs-CRP were the risk fac- tor for prognosis, HDF within a week for protective factors. Conclusions High levels of CRP detection value was one of the risk factors for prognosis of patients with ARF, by reducing the hs-CRP values and inflammatory response of the body coude increase the cure rate for ARF patients.
