摘  要:目的探讨T1rho值对椎间盘早期退变的预测价值及其与Pfirrmann分级和T2值的相关性。方法对93例腰背痛患者行腰椎MR检查,采集腰椎矢状位T1WI及T2WI、轴位T2WI、矢状位T1rho及T2-mapping。利用矢状位T2WI对腰椎间盘退变进行Pfirrmann分级,矢状位T1rho及T2-mapping上分别测量NP区和AF区T1rho值及T2值,分析椎间盘T1rho值与Pfirrmann分级及T2值的关系。结果 93例中,PfirrmannⅠ~Ⅴ级腰椎间盘数分别为0、235、179、41、10个。除PfirrmannⅤ级外,其余各级NP区T1rho值均高于AF区(P<0.001);随椎间盘退变程度加重,NP区T1rho值降低(PfirrmannⅣ级和Ⅴ级间除外),PfirrmannⅡ级AF区T1rho值均高于PfirrmannⅢ~Ⅴ级;NP区及AF区T1rho值与退变分级均呈负相关(NP区:r=-0.75,P<0.001;AF区:r=-0.32,P<0.001);PfirrmannⅡ~Ⅲ级腰椎间盘NP区T1rho值与T2值呈中度相关(PfirrmannⅡ级:r=0.46;PfirrmannⅢ级:r=0.44;PfirrmannⅣ级:r=0.16;PfirrmannⅤ级:r=0.04);AF区T1rho值与T2值相关性差(PfirrmannⅡ级:r=0.13;PfirrmannⅢ级:r=0.15;PfirrmannⅣ级:r=0.19;PfirrmannⅤ级:r=0.62)。结论 T1rho对腰椎间盘早期退变具有预测价值,NP区表现明显;椎间盘早期退变过程中,NP区T1rho值与T2值呈中度相关。Objective To explore the predictive value of Tlrho value in early stage of intervertebral disc degeneration, and to analyze its correlation with Pfirrmann grade and T2 value. Methods Totally 93 low back pain patients who under- went lumbar MR examination were involved. Lumbar sagittal TlWI and T2WI, traverse T2WI, sagittal Tlrho and T2- mapping were obtained. Pfirrmann grade of lumbar discs were evaluated on sagittal T2WI. Tlrho and T2 values of NP and AF were measured separately on sagittal Tlrho and T2mapping, then the relationship between Tlrho, Pfirrmann grade and T2 values was analyzed. Results There were 0, 235, 179, 41, 10 lumbar discs of Pfirrmann I , Pfirrmann ]I , Pfirrmann m , Pfirrmann IV and Pfirrmann V , respectively. Except for Pfirrmann V , Tlrho values of NP were higher than those of AF in each disc level (P〈0. 001). With intervertebral disc degeneration aggravated, Tlrho values of NP decreased (except for that between Pfirrmann IV and V ), Tlrho values of Pfirrmann Ⅱ in AF were higher than that of Pfirrmann Ⅲ-V. Negative correlation was found between Tlrho values and Pfirrmarm grade (NP: r=-0.75, P〈0. 001; AF: r=-0.32, P〈0. 001). Tlrho value and T2 values of NP showed moderate correlation in Pfirrmann Ⅱ Ⅲ (Pfirrmann Ⅱ : r=0.46; Pfirrmann UI : r=0.44; Pfirrmann IV grade: r=0.16; Pfirrmann V : r=0.04); Tlrho value and T2 values of AF had poor correlations (Pfirrmann II : r=0.13; Pfirrmann HI: r=0.15; Pfirrmann IV: r^0.19; Pfirrmann V: r=0.62). Conclusion Tlrho has predictive value for detecting early stage of lumbar intervertebral disc degeneration, especially in NP. In early stage of lumbar disc degeneration, Tlrho values of NP are moderately associated with their T2 values.
