摘　　要：随着经济社会的发展,人民生活水平的提高及生活方式的改变,糖尿病、高血压等慢性非传染性疾病(慢病)逐渐成为危害居民健康的主要原因,同时成为重要的社会公共卫生问题之一和医疗费用增长的主要原因^([1])。而现代信息技术已是社会管理、人民日常生活中不可或缺的重要依托。因此,如何利用信息技术做好慢病一体化防治管理将至关重要。着重对如何依托信息技术——区域卫生信息平台,为推行慢病全程关照计划开创新途径进行探讨。With the development of economic society, the improvement of people＇s living standard and the change of lifestyle, chronic non-communicable diseases （chronic disease）, such as diabetes and hypertension, gradually become the main reason endangering the residents＆#39; health. At the same time, they become one of the important social public health problems and the main cause of increase of health care costs. Modern information technology has become the indispensable and important support in the social management and people＆#39;s daily life. Therefore, how to manage integrated prevention and control of chronic disease with information technology will be critical. How to create a new way for practicing chronic disease whole-course care plan relying on information technology-regional health information platform-is discussed about.
