摘 要:目的初步评价256层螺旋CT血管造影术对胸廓内动脉的解剖学显示价值。方法回顾性分析107例拟诊胸主动脉病变患者的血管造影图像,观察两侧胸廓内动脉的走行规律及其解剖分支特征,沿胸骨角及第6肋间隙水平分为近、中、远3段,测量3个节段血管腔内径、两侧胸廓内动脉长度及其分支肌膈动脉内径和至邻近胸骨缘的距离,并进行统计学分析。结果 107例患者两侧胸廓内动脉显示率均为100%。两侧胸廓内动脉中,同侧近、中、远各段内径比较差异有统计学意义(P<0.05),两侧相应节段比较差异无统计学意义(P>0.05),不同性别各段内径间比较差异无统计学意义(P>0.05)。左侧胸廓内动脉较右侧稍长(P<0.05),男性胸廓内动脉较女性长(P<0.05)。两侧肌膈动脉内径比较差异无统计学意义(P>0.05),不同性别间内径比较差异无统计学意义(P>0.05)。至胸骨边缘距离,两侧比较差异无统计学意义(P>0.05),不同性别间距离比较差异无统计学意义(P>0.05)。两侧胸廓内动脉末端分支,右侧两支分支者89例(83.18%)、三支者10例(9.35%)、四支者8例(7.48%)。左侧两支分支者91例(85.05%)、三支者9例(8.41%)、四支者7例(6.54%),两侧均以两支分支型最多见。结论采用256层螺旋CT血管造影术可以清晰观察胸廓内动脉走行规律及其解剖分支特征,可为临床提供较为重要的解剖信息,对术前评估有一定临床意义。Objective To study the value of 256 slice spiral CT(MSCT) in displaying anatomy of internal thoracic ar- tery. Methods The angiographic images of 107 patients who were suspected of thoracic aortic disease were retrospective- ly analyzed. The running rule and branches of internal thoracic artery on both sides were observed, Each side of the internal thoracic artery were divided into 3 sections according to the level of sternal angle and the sixth intercostal space, then lumen diameter, vessel length and its branches of three segmental vascular of the internal thoracic artery were measured. The lu- men diameter of the diaphragmatic artery diameter, the distance between internal thoracic artery and each border of the ster- num were measured. Results 107 patients with both sides of the internal thoracic artery can be shown( 100% ). the vas- cular lumen diameter of intrathoracic artery had no statistical difference between left and right side ( P 〉 0.05 ). The left internal thoracic artery was longer than right side ( P 〈 0.05). The intrathoracic artery of male was longer than that of fe- male ( P 〈 0.05 ). There was no statistical difference of diaphragmatic arteries diameter between two sides ( P 〉 0.05 ). The terminal branch type of internal thoracic artery included two branches (83.18%), three branches (9.35%), four branches (7.47%) on the left side and two branches (85.05%), three branches (8.41%), four branches (6.54%) on left side Conclusion The application of 256 slice spiral CT angiography can show the anatomical features of internal tho- racic artery clearly, and provide a relatively important information for clinical.
