人工肱骨头置换治疗肱骨近端复杂骨折并骨质疏松老年患者的临床疗效

摘　　要：[目的]回顾分析人工肱骨头置换和解剖锁定钢板在肱骨近端复杂骨折并骨质疏松老年患者的应用，探讨人工肱骨头置换在临床上的效果。[方法]将2006年6月～2011年6月期间收治的老年骨质疏松肱骨近端复杂骨折进行分组：人工肱骨头置换组（28例）、解剖锁定钢板组（2l例）；术中详细记录两组的术中出血量、手术时间、术后引流量、手术前后血红蛋白量。记录两组术后6、12个月肩关节功能的Neer评分。[结果]人工肱骨头置换组和解剖锁定钢板组术中出血量、手术时间、术后引流量、术后血红蛋白量两组有显著性差异（P〈0．05），术后6、12个月肩关节功能的Neer评分有显著性差异（P〈0．05）；术前血红蛋白量两组无统计学意义（P〉0．05）。[结论]治疗老年骨质疏松肱骨近端复杂骨折，人工肱骨头置换术手术时间短、出血少、近期肩关节功能恢复好，临床效果满意。[ Objective] To restrospectivey analyze the effect of humeral head replacement and anatomical locking plate in elderly patients with complex proximal humerus fractures and osteoporosis, and to discuss the clinical effects of humeral head re- placement. [ Method ] From June 2006 to June 201 !, elderly patients of the osteoporotic proximal humeral fracture were divided into two groups ： humeral head replacement group （ 28 cases ）, anatomical locking plate group （ 21 cases ）. Details of the two groups of bleeding volume, operation time, postoperative drainage volume, the amount of hemoglobin before and after operation were recorded. Neer score of 6,12 months of shoulder joint function after operation in two groups were recorded. [ Result] A- mount of bleeding, operation time, postoperative drainage volume, postoperative hemoglobin content showed significant difference between two groups （ P 〈 0.05 ）. After 6 to 12 months, the Neer score had no significant difference （ P 〈 0.05 ）. Preoperative he- moglobin of two groups had no statistical significance（ P 〉 0.05 ）. [ Conclusion] In the treament of elderly patients of the osteo- porotic proximal humeral fractures, humeral head replacement can improve the shoulder joint function in short time and require satisfactory clinical effect.
