摘 要:【摘要】目的观察羟氯喹对SLE患者PBMCs凋亡及辅助性T细胞(Th)17、IL-17因子表达水平的影响。方法选取20例初发活动期SLE患者,提取PBMCs进行培养,用羟氯喹、苯丁酸氮芥(CLB)2种药物进行干预,培养24、48h用流式细胞术检测PBMCs凋亡及Thl7细胞表达水平的影响;提取细胞上清液用ELISA检测IL-17因子的表达。采用单因素方差分析和SNK—q检验进行统计学处理。结果单个核细胞凋亡率:24h羟氯喹(10_3±0.7)%、CLB(8.5±1.1)%;48h羟氯喹(13.9±0.6)%、CLB(11.8±0.8)%,2组24、48h比较差异均有统计学意义(P〈O.05);Th17细胞的百分数:24h羟氯喹(0.81±0.13)%、CLB(0.78±0.26)%,48h羟氯喹(0.73±0.45)%、CLB(0.68±0.20)%,2组24、48h比较差异均有统计学意义(P〈0.05);IL-17:24h羟氯喹(263±0.9)%、CLB(24.6±0.7)%;48h羟氯喹(24.2±0.9)%、CLB(22.6±1.1)%,2组24、48h比较差异均有统计学意义(火0.05)。结论通过体外实验证明羟氯喹有诱导PBMCs凋亡的作用,羟氯喹降低PBMCs中Thl7细胞水平,减少外周血中IL-17因子的分泌。Objective To observe the effect of Hydroxyehloroquine (HCQ) on the apoptosis of peri- pheral blood mononuclear cells (PBMCs) of Systemic lupus erythematosus (SLE) patients and on the level of Thl7 cells and IL-17. Methods The peripheral blood of 20 incipient SLE patients in active stage were taken, and PBMC were separated for cell culture. Using HCQ and Chlorambucil (CLB) as an intervention, and after cultured for 24 h and 48 h, the apoptosis of PBMC an(] the level of Thl7 were tested using Flow cytometry (FCM), the supematants were collected to test for the level of cytokine IL-17 by ELISA. One-way ANOVA was used and SNK-q was used in the comparison between every two groups. Results There was significant difference in the apoptosis rate of mononuc]ear cells between the HCQ and CLB group at 24 h [HCQ: (10.3±0.7)%, CLB: (8.5±1.1)%1 and 48 h [HCQ: (13.9±0.6)%, CLB: (11.8±0.8)%] (P〈O.05). There was significant difference between HCQ [24 h: (0.81±0.13)%, 48 h: (0.73±0.45)% ] and CLB group [24 h: (0.78±0.26)%, 48 h: (0.68±0.20)%1 in Thl7 percentage (P〈0.05). The levels of IL-17 in the supernatants of the HCQ group [24 h: (26.3±0.97)%, 48 h: (24.2±0.91)%] and CLB group [24 h: (24.6± 0.7)%, 48 h: ( 22.6± 1.1 )% ] were significantly different between the two groups(P〈0.05 ). Conclusion HCQ has apoptosis-induetiou effect on PBMC, and it can decrease the number of Thl7 and IL-17 level in the PBMCs.
