摘　　要：目的观察左心衰竭(LHF)患者血浆可溶性Fas(sFas)的水平变化并探讨临床意义。方法入选LHF患者35例为LHF组,健康体检者20例为对照组。采用纽约心脏病学会(NYHA)心功能分级标准对LHF患者心功能进行分级。采用酶法检测两组血清肌酐(sCr)、丙氨酸氨基转移(ALT)、血糖(BG)和脑利钠肽(BNP)。ELISA法检测两组血浆sFas水平。结果 LHF组血清BNP、BG、sCr及血浆sFas水平均高于对照组(P均<0.05)。LHF组心功能Ⅱ级、Ⅲ级和Ⅳ级者血浆sFas水平分别为(533.62±261.55)、(860.54±292.50)、(1 206.22±510.98)pg/mL,三者相比,P均<0.05;相关性分析结果显示LHF患者血浆sFas水平与心功能分级呈正相关关系(r=0.614,P=0.000)。结论 LHF患者血浆sFas水平高于正常,且与心功能分级呈正相关;sFas水平变化可能与LHF的发病有关。Objective To investigate clinical significances of changes in levels of plasma soluble Fas （sFas） in patients with left heart failure （L HF）.Methods The LHF group of 35 patients and the control group of 20 healthy persons were recruited in the study.Patients in the LHF group were evaluated for their cardiac function according to New York Heart Association （NYHA） functional classification.Patients in the two groups were detected for serum creatinine （sCr）,alanine aminotransferase （ALT）,blood glucose （BG） and brain natriuretic peptide （BNP） by the enzymic method,with plasma sFas detected by enzyme linked immunosorbent assay （ELISA）.Results Concentrations of BNP,BG,sCr and sFas in the LHF group were significantly higher than those in the control group （all P ＜ 0.05）.In the LHF group,the plasma sFas levels of different cardiac function of class Ⅱ,class Ⅲ and class Ⅳ were （533.62 ±261.55） pg/mL,（860.54 ±292.50） pg/mL and （1 206.22 ±510.98） pg/mL,respectively,and there were significant differences among all subgroups （all P ＜0.05）.Spearman rank correlation showed a positive correlation between severity of LHF and plasma sFas （r =0.614,P =0.000）.Conclusions Plasma sFas levels could be significantly increased in LHF patients and show a positive correlation with severity of LHF.Therefore,the changes in levels of plasma sFas may be related with occurrence of LHF.
