捏脊疗法结合神经生理疗法治疗弛缓型脑瘫的临床疗效及机制研究

摘　　要：目的研究捏脊疗法结合神经生理疗法对弛缓型脑瘫的治疗作用及作用机制。方法将60例弛缓型脑瘫患儿随机分为2组,观察组30例采用捏脊疗法结合神经生理疗法治疗,对照组30例采用神经生理疗法。治疗前、后采用粗大运动功能评定量表(GMFM)-66评估2组患儿的运动功能,同时检测治疗前后2组患儿血中的微量元素水平。结果观察组总有效率96.67%,对照组76.66%,观察组疗效显著优于对照组,2组比较差异有统计意义(P<0.05)。观察组治疗后微量元素Cd,Pb水平与治疗前比较有所降低,其中Pb下降显著,与同组治疗前及对照组治疗后比较差异有统计意义(P<0.05);观察组治疗后Cu,Zn,Ca,Fe,Mg,Mn水平与治疗前比较有不同程度升高,其中Cu,Ca升高显著,与同组治疗前及对照组治疗后比较差异有统计意义(P<0.05)。对照组治疗前后微量元素变化不明显,差异均无统计意义(P>0.05)。结论捏脊疗法能显著提高弛缓型脑瘫的治疗效果,推测其作用机制可能是捏脊疗法对特定部位的刺激作用以及促进患儿的消化吸收功能改善了机体的营养状态,从而促进了大脑等神经系统和运动系统的发育。Objective To study the effect and mechanism of chiropractic therapy combined with neural physiological therapy on hypotonia cerebral palsy and its mechanism of action. Methods 60 cases of hypotonia cerebral palsy children were randomly divided into 2 groups, the observation group （30 cases） was treated with chiropractic therapy combined with neural physiological therapy, the control group （30 cases） was treated with electrophysiological therapy. The gross motor function assessment scale （GMFM）-66 and blood trace element level in 2 groups were detected before and after treatment. Results The total effective rate was 96.67% in the observation group, 76.66% in the control group, the curative effect of therapeutic group was significantly better than the control group, the difference between the 2 groups had statistical significance （ P〈0.05 ）. Trace elements Cd, Pb levels of the observation group after treatment decreased compared with those before treatment. Pb decreased significantly after treatment compared with the same group before treatment and control group （ P〈0.05） ; Cu, Zn, Ca, Fe, Mg, Mn levels of the observation group after treatment increased differently, while Cu, Ca increased signifi- cantly, there was statistical difference before treatment and after treatment or compared with control group （P〈 0. 05 ）. Trace elements of the control group before and after treatment did not change significantly, there was no statistical significance （ P 〉 0.05 ）. Conclusion Chiropractic therapy can significantly improve the therapeutic effect of hypotonia cerebral palsy. The mechanism may be stimulating effect of chiropractic therapy on specific sites and promote digestion and absorption function, improve the body＇ s nutritional status, so as to promote the brain and nervous system and motor system development.
