兔膝退变软骨MRI与软骨内基质金属蛋白酶-13(MMP-13)表达的研究

摘  要:目的观察模型兔退变软骨MRI表现及基质金属蛋白酶-13(MMP-13)的表达,探讨两者关系,研究MMP-13在退变诊断中的价值。方法健康新西兰大白兔40只随机分两组。右后膝行前交叉韧带切断。左后膝为对照。术后4周和8周,观察软骨退变MRI表现并进行MMP-13染色。结果术后4周,软骨以II级退变为主(55%);术后8周,软骨以III级退变为主(60%),差异具有统计学意义(P<0.05)。对照侧MMP-13轻度表达,主要在软骨表层,术后4周高度表达为主(85%),主要在表层和中层,差异具有统计学意义(P<0.01);术后8周较术后4周下降,以中度表达为主(50%),仍高于对照组,差异具有统计学意义(P<0.01)。I级退变软骨MMP-13轻度表达为主(5/6),II级高度表达为主(13/17),III级中度表达为主(10/17),两者存在相关性(r=0.631,P<0.01),II级前,随分级加重,MMP-13表达增高;II级后,随分级加重,MMP-13表达降低。结论MMP-13的表达与退变软骨MRI分级具有相关性,检测其表达对发现早期退变具有一定意义。Objective To observe MMP-13 and MRI of cartilage degeneration, then to investigate the correlation between them and evaluate the role of MMP-13 in the development of degenerative osteoarthropathy(DO). Methods Forty healthy adult New Zealand rabbits were departed into 2 groups stochastically. Right stifle-joints were selected as operation, left as control. The MRI and MMP-13 in different stages were observed and graded. Results Degenerative cartilage was mainly grade II at 4 weeks; mainly grade III at 8 weeks. There was significant difference between them(P〈0.05). The MMP-13 in control was light, predominant at 4 weeks, there was sig nificant difference(P〈0.01); It was lower at 8 weeks than at 4 weeks, there was significant difference (P〈0.01). There was correlation between the expression of MMP-13 and the grade of MRI(r=0. 631, P〈0.01). Conclusion There is statistical correlation between MMP-13 and the MRI grade, to detect MMP-13 has an important significance in diagnosis of cartilage degeneration in the early stage.
