桂枝茯苓丸方对孕三烯酮胶囊治疗子宫腺肌病的增效作用

摘　　要：目的观察桂枝茯苓丸方治疗子宫腺肌病(AM)的临床疗效。方法 205例AM患者随机分为中药组52例、对照组50例、中西医结合Ⅰ组51例及中西医结合Ⅱ组52例。中药组予以桂枝茯苓丸方,每日1剂;对照组在月经期的第1-5天内口服孕三烯酮胶囊,每周一、四或二、五口服1粒,每粒2.5 mg,3个月后开始服用中药安慰剂;中西医结合Ⅰ组服用孕三烯酮胶囊开始即服用桂枝茯苓丸方;中西医结合Ⅱ组服用孕三烯酮胶囊3个月后开始服用桂枝茯苓丸方。各组均治疗6个月后观察临床疗效,于治疗前及治疗后3、6个月测定子宫体积、血清CA125含量及痛经评分,观察各组治疗中副作用及妊娠结局。结果中药组、对照组、中西医结合Ⅰ组、中西医结合Ⅱ组临床疗效总有效率分别为92.3%、66.0%、92.2%、92.3%,其他3组均优于对照组(P<0.01)。除中药组子宫体积外各组治疗后3个月与本组治疗前子宫体积、CA125、痛经评分比较差异均有统计学意义(P<0.05);除中药组子宫体积外,中药组和中西医结合Ⅰ、Ⅱ组治疗后6个月各指标均优于对照组(P<0.05)。中药组无明显副作用,与其他3组比较差异有统计学意义(P<0.05)。结论桂枝茯苓丸方治疗AM在缓解疼痛、降低血清CA125、有效控制子宫体积方面疗效确切,对孕三烯酮胶囊的治疗有增效作用。Objective To observe the therapeutic effect of Guizhi Fuling Pill （桂枝茯苓丸） on adenomyosis （AM）. Methods Totally 205 cases of AM were randomized into the Chinese medicine group of 52 cases, control group of 50 cases, integrative medicine group I of 51 cases and integrative medicine group II of 52 cases. The Chi- nese medicine group was given Guizhi Fuling Pill （桂枝茯苓丸） one dose daily. The control group was given Gestri- none capsules from the 1 st to 5th day during the menstruation and given placebo 3 months later. The integrative medi- cine group I was given Guizhi Fuling Pill （桂枝茯苓丸） at the beginning of administration of Gestrinone capsules. The integrative medicine group I1 was given Guizhi Fuling Pill （桂枝茯苓丸） after 3-month administration of Gestri- none capsules. The therapeutic effect was observed after 6-month treatment. The uterine volume, serum CA125 level and dysmenorrhea score was detected before and after treatment. The side effects and pregnancy outcome were ob- served after treatment. Results The total effective rate of the Chinese medicine group, control group, integrative medicine Ⅰ group and integrative medicine group Ⅱ was 92. 3%, 66.0%, 92.2% and 92.3% respectively, with a significant difference between control group and other three groups （P 〈 0. 01 ）. There were significant differences be- tween before and after 3-month treatment in the uterine volume, serum CA125 level and dysmenorrhea score in all groups except the uterine volume in Chinese medicine group （ P 〈 0.05 ）. All indicators in the Chinese medicine group, integrative medicine group I and integrative medicine group Ⅱ were superior to those in the control group af- ter 6-month treatment except the uterine volume in Chinese medicine group （P 〈 0. 05 ）. There was no obvious ad- verse reaction in the Chinese medicine group, with a significant difference between other 3 groups （P 〈 0.05 ）. Conclusion Guizhi Fuling Pill （桂枝茯苓丸） is effective
