上海市110例不宜继续妊娠案例干预效果的分析

摘　　要：目的：探讨不宜继续妊娠孕妇的管理模式及其结果，以提高对高风险孕妇的保健服务质量，进一步降低孕产妇和围生儿死亡率。方法：回顾性分析2009～2011年上海市各区县上报的110例不宜继续妊娠孕妇的相关管理资料，由社区卫生服务中心将有异常情况和体征的孕妇进行初筛，并及时转诊至二、三级助产医疗机构确诊，并密切随访至产后42天为止。结果：经过社区-妇幼保健所-医院全程、持续、联动的干预下，110例患者中有74．55％及时终止妊娠，25．45％在医生的监护下继续妊娠并顺利分娩。结论：上海市不宜继续妊娠孕妇的管理取得了一定的成效，建议进一步规范管理，以降低孕产妇和围产儿死亡率。Objective： To explore the management model and outcome of women with diseases unsuitable for pregnancy, so as to improve the quality of healtheare service for high risk pregnant women and further reduce the mortality rates of pregnant women and perinatal infants. Methods： The relevant management data of 110 women with diseases unsuitable for pregnancy reported by districts and counties of Shanghai city from 2009 to 2011 were analyzed retrospectively; the pregnant women with abnormal situations and signs were screened in community health service center for the first time, and then the pregnant women were transferred to Grade two and three midwifery medical institutions timely for definite diagnosis, all the pregnant women were followed up closely until 42 days after delivery. Results： After thoroughly, continuous and jointly intervention from community to maternal and child health care institute and hospital, 74. 55% of the 110 patients terminated pregnancy timely, and 25.45% of them continued pregnancy and gave birth to their babies smoothly under monitoring of doctors. Conclusion： The management on the women with diseases unsuitable for pregnancy have taken effects in Shanghai city. It is advised to further standardize the management and reduce the mortality rates of pregnant women and perinatal infants.
