摘　　要：目的：比较左炔诺孕酮宫内缓释系统（LNG—IUS）与宫内含铜节育器（TCu380A）应用2年对骨密度的影响。方法：选取2008年～2010年期间，放置LNG—IUS者48例，放置宫内含铜节育器者（TCu380A）36例。患者年龄在40～45岁期间，两组均在放置前及放置后2年测量股骨颈及腰椎骨密度。同时监测放置前及放置后2年骨钙蛋白及血清吡啶啉指标。结果：观察2年LNG—IUS组与TCu380A组在同一年龄段、体质指数无明显差异下，股骨颈及腰椎骨密度无统计学差异。骨钙蛋白及血清吡啶啉指标也无统计学差异（P〉0．05）。结论：使用LNG～IUS组2年后对骨密度、骨钙蛋白及血清吡啶啉水平与使用TCu380A组无统计学差异，表明LNG—IUS使用2年对骨密度无不利影响。Objective： To compare the effects of levonorgestrel - releasing intrauterine system （ LNG - IUS） and TCu380A on bone mineral density （BMD） after applying for two years. Methods： The cases treated with LNG -IUS （48 cases） and TCu380A （36 cases） were selected from 2008 to 2010; the patients were 40 -45 years old, BMD values of collum femoris and lumbar vertebra were detected before treatment and at two years after treatment in the two groups; meanwhile, osteocalcin and serum pyridinoline levels were detected. Resuits ： There was no statistically significant difference in BMD values of collum femoris and lumbar vertebra, osteocalcin and serum pyridinoline levels at two years after treatment between the two groups in the same age group （P 〉 0. 05 ） . Conclusion ： This study showed that LNG -：IUS used for two years has no adverse impact on BMD.
