摘 要:目的评价深圳市某社区15岁以下人群规范全程接种乙肝疫苗1年后的免疫水平。方法对某社区经检测HBsAg阴性的15岁以下人员按"0、1、6"程序接种乙肝疫苗,符合全程规范接种要求的1 986例对象在接种疫苗1年后进行抗-HBs定性和定量检测。结果 (1)在该小区建立有健康档案的15岁以下各组人群乙肝疫苗全程接种率均在95%以上,平均为96.18%。(2)该1 986名研究对象在接种乙肝疫苗1年后检测抗-HBs,平均抗体阳性率为93.06%,抗-HBs几何平均浓度为267.89 mIU/ml。新生儿组、学龄前组、小学组和初中组抗-HBs阳性率分别为96.46%,94.32%,92.75%和90.02%;平均抗体浓度分别为312.17 mIU/ml,290.33 mIU/ml,265.13 mIU/ml和218.44mIU/ml,组间差异有统计学意义(P<0.05)。随着疫苗接种年龄的增大,抗-HBs阳性率和抗体浓度有下降趋势。(3)新生儿组、学龄前组、小学组和初中组抗-HBs阳性率和平均抗体浓度男性之间和女性之间组间差异有统计学意义(P<0.05)。但同年龄组男女性之间差异无统计学意义(P>0.05)。结论 15岁以下人员接种乙肝疫苗免疫效果良好,年龄越小接种,保护性作用越大。Objective To evaluate immunization effect of hepatitis B vaccine after one year inoculation among children under the age of 15. Methods Anti - HBs qualitative and quantitative detection were implemented in the 1 986 cases who were inoculated hepati- tis B vaccine one year ago according to the "0, 1, 6" program. Results (1) The rate of hepatitis B vaccine inoculation was 96.18 per- cents among the young people under 15 whose health files were established in the resident community. (2) The anti - HBs average antibody positive rate was 93.06 percent and geometric mean concentration was 267.89 mIU/ml in the 1 986 participators. The study objects were divided into four groups (the neonatal, preschool, primary and junior middle school student groups) and the anti-HBs positive rates were respectively 96.46, 94.32, 92.75 and 90.02 percents, and the average antibody concentration were respectively 312.17 mIU/ml, 290.33 mIU/ml, 265.13 mIU/ml and 218.44 mIU/ml, and the significant difference was found (P〈0.05). Further more, the results indicated that the anti - HBs positive rate and antibody concentration declined with the increase of age when vac- cination. (3) The boys' and girls' positive rate of anti - HBs and antibody concentrations in the neonatal, preschool, primary and ju- nior middle school student groups were different between groups (P〈O.05), but which in the same age groups were not different (P〉 0.05). Conclusion Immune effect is good by inoculating hepatitis B vaccine among children under 15. The smaller the age of inocu- lation is, the greater the protective effect is.
