缬沙坦氨氯地平片对高血压合并非酒精性脂肪肝患者动脉弹性的影响

摘　　要：目的 :探讨缬沙坦氨氯地平片降压达标治疗对高血压合并非酒精性脂肪肝(NAFLD)患者动脉弹性的影响。方法 :收集高血压合并NAFLD患者175例,随机将患者分为观察组[缬沙坦氨氯地平片(缬沙坦80 mg,氨氯地平5 mg)]90例,对照组[厄贝沙坦氢氯噻嗪分散片(厄贝沙坦150 mg,氢氯噻嗪12.5 mg)]85例,每日1片,据血压调整剂量,可合并利尿剂及β受体阻滞剂;并严格执行生活干预,观察时间共6个月。结果 :本研究初步表明观察组和对照组治疗后血压均有明显下降(P<0.01);两组之间血压下降、单药达标率差异无统计学意义(P>0.05);两组治疗后体重指数、血脂水平均较治疗前降低(P<0.05),组间差异无统计学意义(P>0.05);两组治疗后动脉弹性指数及颈动脉内中膜厚度均有明显下降(P<0.01),观察组下降幅度更大,明显优于对照组(P<0.05)。结论 :对高血压伴NAFLD患者,积极降压会延缓动脉硬化进程;缬沙坦氨氯地平片和厄贝沙坦氢氯噻嗪分散片在降压的同时,皆有改善动脉硬化及逆转颈动脉内中膜厚度的作用,且前者优于后者。Objective： To explore the effect of valsartan-amlodipine tablet on arterial elasticity in patients with essential hypertension （EH） combining non-alcoholic fatty liver disease （NAFLD）. Methods： A total of 175 patients with EH combining NAFLD were divided into 2 groups, Observation group, the patients received valsartan （80mg）-amlodipine （5mg） tablet, n=90 and Control group, the patients received irbesartan （150mg） -hydrochlorothiazide （12.5mg）, n=85. The medication was combined with diuretics and 13-receptor blocker, the dosage was adjusted by blood pressure and the life style was strictly watched. All patients were treated for 6 months and the effects were compared between 2 groups. Results： After treatment, the blood pressure was significantly decreased in both groups, P〈0.01 and the deduction rates were similar between 2 groups,P〉0.05; the BMI and blood lipids were decreased in both groups, P〈0.05 and the effects were similar between 2 groups, P〉0.05. After treatment, the arterial elasticity index and carotid intima-media thickness were decreased in both groups, P〈0.01, while the effect in Observation group was superior to Control group, P〈0.05. Conclusion： Anti-hypertension treatment may improve the arteriosclerosis in patients with EH combining NAFLD.Both valsartan-amlodipine tablet and irbesartan-hydrochlorothiazide could reduce the carotid intima-media thickness, while valsartan-amlodipine tablet had the better effect.
