摘 要:目的探讨磁共振扩散加权成像(DWI)在子宫内膜病变中的临床应用价值,通过对表观扩散系数(ADC)值的分析,对子宫内膜良恶性病变进行鉴别。方法对30例经病理证实的子宫内膜病变患者进行常规MRI(磁共振成像)扫描及DWI扫描。其中子宫内膜癌18例,子宫内膜良性病变12例,分别测量病灶的ADC值,并进行对比分析。结果子宫内膜癌和子宫内膜良性病变的ADC值分别为(0.88±0.11)×10-3mm2/s和(1.19±0.13)×10-3mm2/s,两者相比具有统计学差异(P<0.01);以1.00×10-3mm2/s作为诊断子宫内膜良恶性病变的阈值,其敏感性、特异性和准确性分别为88.9%、100%和93.3%。结论 DWI中,通过ADC值的测量能够初步鉴别子宫内膜良恶性病变。Objective To evaluate the effectiveness of DW MRI (Diffusion-Weighted Magnetic Resonance Imaging) in differential diagnosis of uterine endometrial lesions through analysis of the ADC (Apparent Diffusion Coefficient) value. Methods Among 30 pathologically-confirmed uterine endometrial lesion patients undergoing conventional MR/and DWI (Diffusion Weighted Imaging), there were 18 cases of uterine endornetrial carcinoma and 12 benign endometrial lesions. Based on the ADC values measured in all the lesions, comparisons were made respectively. Results The ADC value of uterine endometrial carcinoma was (0.88~0.11)~ 10-3mm2/s versus benign endometrial lesions' (1.19~0.13)x 10-3mm2/s with statistical difference (P〈0.01) between them. When 1.00x 10-3mm2/s was taken as the threshold for differential diagnosis of benign (〉 1.00 ~ 10-3mm2/s) and malignant (〈 1.00 ~ 10-3mm2/s) uterine endometrial lesions, the sensitivity, specificity and accuracy of malignant uterine endometrial lesions were 88.9%, 100% and 93.3% respectively. Conclusion ADC measurement in DWI allowed for preliminary differentiate diagnosis of uterine endometrial lesions.
