摘 要:目的探讨乳腺癌患者能量多普勒超声血流定量与肿瘤组织VEGF、TNF-α及TGF-β1表达的相关性。方法选取本院收治、预行手术治疗的21例乳腺癌患者及44例乳腺良性瘤患者为研究对象,术前以能量多普勒测定肿瘤血流的彩色象素密度;术后取病理组织及瘤体周边正常组织,制备石蜡切片,以免疫荧光法测定VEGF、TNF-α及TGF-β1的表达。结果多普勒测定结果显示,乳腺癌患者的彩色象素密度显著高于乳腺良性瘤患者(P<0.05);免疫荧光测定结果显示,VEGF、TNF-α及TGF-β1的表达趋势一致,以乳腺癌组织为最高,良性瘤组织次之,瘤体周边正常组织最低,各组之间差异显著(P<0.05)。经线性相关分析,乳腺癌患者的VEGF、TNF-α及TGF-β1表达量均与其彩色象素密度存在直线正相关的关系,而乳腺良性瘤患者仅VEGF表达量与其彩色象素密度呈现出一定的相关性,TNF-α及TGF-β1两项指标则无显著相关性。结论乳腺癌患者能量多普勒超声血流定量值显著高于乳腺良性瘤患者,且与肿瘤组织VEGF、TNF-α及TGF-β1表达均呈现出显著相关性,可能为其区别于良性乳腺肿瘤的重要机制之一。With high morbidity and mortality, breast cancer has attracted more attention in resent years. It is believed that early diagnosis is the key point directly related to patient survival. Here, we aim to explore the correlation of color pixel density detected by contrast-enhanced power doppler with the expression of VEGF, TNF-α, and TGF-β1, so as to give light to imaging diagnosis of breast carcinoma. Total of 21 cases of breast cancer patients as well as 44 cases of breast carcinoid were selected and analyzed, in which color pixel density of the tumor were detected by contrast-enhanced power doppler before surgery, and pathological tissue as well as healthy tissue of breast were prepared for paraffin section to detect the expression of VEGF, TNF-α, and TGF-β1 with immunofluorescence. Doppler measurement results showed that patients with breast cancer got higher color pixel density compared with those with breast carcinoid (P 〈 0.05); immunofluorescence results showed that expression trends of VEGF, TNF-α, and TGF-β1 were accordant: the breast cancer tissue the highest, breast carcinoid tissue the second, and the healthy tissue the lowest (P 〈 0.05). Linear correlation analysis results showed that the expression of VEGF, TNF-α and TGF-β1 were all positively linear related with the color pixel density in patients with breast cancer; while for patients with breast careinoid, there was only VEGF expression showed linear positive correlation with the color pixel density. Our research results indicated that the color pixel density detected by contrast-enhanced power doppler in patients with breast cancer is significantly higher than that of patients with breast carcinoid, and shows significant correlation with the expression of VEGF, TNF-α, and TGF-β1 in tumor tissues, which may be one of the important mechanisms for breast cancer diagnosis.
