摘 要:目的 探讨心电脉冲窗的优化对胸痛三联排查(triple-rule-out,TRO)图像质量及辐射剂量的影响.方法 将80例拟行回顾性心电门控TRO的患者依据心率及心电脉冲窗的范围分为A1(心率<70次/min; 30%~70% R-R间期)、B1(心率<70次/min;60%~75%R-R间期)、A2(心率>80次/min;30%~70% R-R间期)、B2(心率>80次/min;30%~50% R-R间期)4组,每组20例.2名研究者采用4分法评价4组图像质量.记录剂量长度乘积(DLP),计算有效剂量(ED).采用t检验比较A1与B1、A2与B2组的图像质量和辐射剂量.结果 A1与B1组、A2与B2组图像质量评分差异均无统计学意义(P>0.05).B1组有效剂量较A1组降低约28.50%,B2组有效剂量较A2组降低约16.01%,差异均有统计学意义(P<0.05).结论 心电脉冲窗的优化可以获得满足诊断要求的图像,且辐射剂量明显降低,在TRO低剂量扫描中具有重要价值.Objective To investigate the effect of optimized ECG-pulsing window on image quality and radiation dose at triple rule-out CT angiography (TRO-CTA).Methods 80 patients requiring TRO-CTA were averagely assigned to four groups according to HR and the range of ECG-pulsing window:group A1 (HR<70 beats/min and 30% ~70% R R interval),group B1 (HR<70 beats/minand 60% ~75% R-R interval),group A2 (HR>80 beats/min and 30% ~70% R-R interval) and group B2 (HR>80 beats/min and 30% ~50% R-R interval).Image quality was assessed by two observers using a four-point scale.Estimation of effec tive dose (ED) was performed on the basis of dose length product (DLP).Image quality and radiation dose were compared with t test between A1and B1,A2 and B2,respectively.Results There was no difference on image quality between A1 and B1,and between A2 and B2,but the effective dose was decreased by 28.50%for B1 and by 16.01%for B2 using optimized ECG-pulsing window.Conclusion Optimized ECG-pulsing window can provide diagnostic image quality at reduced radiation dose and has a great value in low-dose TRO-CTA.
