摘  要:目的探讨健康体检人群中,14种食物不耐受的检测现状及其与人体泌尿结石的相关性。方法选取2011年1月至2012年12月某综合医院健康管理中心进行食物不耐受检测者490例,采用酶联免疫吸附法检测血清14种食物过敏原特异性IgG抗体,采用x2检验比较食物不耐受不同程度分级及每一种食物不耐受阳、阴性组人体泌尿结石的情况,应用非条件Logistic回归模型进行泌尿结石的多因素分析。结果按IgG抗体阳性率高低排序,前三位分别为鸡蛋(32.45%)、螃蟹(27.96%)、牛奶(12.65%)。存在食物不耐受的检测者中,轻、中、重度食物不耐受分别占67.15%、24.09%、8.76%,中、重度不耐受多见于鸡蛋、牛奶,其次为螃蟹和鳕鱼。在控制收缩压、年龄、体重、红细胞等混杂因素的影响后,食物不耐受是泌尿结石的保护因素(OR=0.965,95%CI:0.943~0.987),且存在一定的剂量一反映关系,食物不耐受程度越重,结石阳性率越低(x2=7.191,P=0.007);牛奶、鸡蛋两种食物不耐受阳性组的泌尿结石阳性率均显著低于不耐受阴性组(X2=5.370,P=0.020;x2=6.696,P=0.010),其余12种食物不耐受阳性组与不耐受阴性组的泌尿结石阳性率差异均无统计学意义。结论食物不耐受情况较为普遍。食物不耐受并非加重人体泌尿结石的发病率,而是与泌尿结石呈负相关,这可能与人们因食物不耐受而限制食物的摄取有关联。Objective To investigate the prevalence of 14 kinds of food intolerance and to determine the association between food intolerance and human urinary stones. Methods 490 cases of food intolerance from the Heahh Management Center of a general hospital were selected in January 2011 to December 2012. Serum levels of 14 kinds of food-specific IgG antibodies were measured using ELISA method. Chi-square test was used to analyze the incidence rate of human urinary stones between groups with different degrees of food intolerance. Unconditional logistic regression was used to determine the effect of food intolerance on urinary stones. Results The top three intolerant foods were eggs (32.45%), crabs (27.96%) and milk (12.65%). The proportion of mild, moderate and severe food intolerance rate were 67.15%, 24.09% and 8.76%, respectively. Most of the moderate to severe intolerance cases were related to eggs and milk, then crab and cod. After controlling of the systolic blood pressure, age, body weight, red blood cells and other confounding factors,food intolerance was negatively associated with human urinary stones (OR=0.965,95%CI:0.943-0.987).A negative dose-response relationship was revealed, that is, the positive rate of human urinary stones decreased as the severity levels of food intolerance increased (~2=7.191 ,P=O.007). Particularly, the positive rate of human urinary stones was significantly lower in subjects with food intolerance of milk and egg comparing with those without food intolerance of milk and egg (X2= 5.370, P=0.020;X2=6.696, P=0.010). There was no significant difference between the positive rate of urinary stones and the food intolerant on the rest of the other 12 kinds of foods. Conclusion Food intolerance is common and is negatively associated with human urinary stones, which may be associated with restricted food intake due to food intolerance.
