沙眼衣原体相关抗体在筛查输卵管性不孕中的应用探讨

摘　　要：目的通过研究沙眼衣原体抗体IgG(CT-IgG)与衣原体热休克蛋白60-IgG抗体(chsp60-IgG)两种沙眼衣原体相关抗体与输卵管病变的关系,探讨更准确的输卵管性不孕不育的初筛指标。方法收集经腹腔镜确诊的输卵管性不孕不育症患者空腹血清标本55例,健康人空腹血清标本116例,采用酶联免疫吸附法和免疫比浊法分别对两组衣原体相关抗体CT-IgG与chsp60-IgG进行检测。计算CT-IgG和chsp60-IgG单个指标,两者联合共3种筛查方法的阳性预测价值、阴性预测价值、灵敏度、特异度,阳性似然比、阴性似然比和约登指数(Youden)等。结果两组沙眼衣原体抗体IgG检测阳性率分别为54.55%和7.76%,两组chsp60-IgG检测阳性率分别为40.00%和22.41%。两种衣原体抗体联合检测在两组人群中的阳性率分别为87.27%和24.1%。CT-IgG在两组中的OR值为14.3,CT-IgG和chsp60-IgG两者联合检测在两组中的OR值为21.6,差异有统计学意义(P<0.05)。结论 CT-IgG和chsp60-IgG联合检测在预测输卵管性不孕中具有较高的筛查价值。Objective To investigate the correlation between the pathological changes of oviduct and the antibodies for chlamydia trachomatis （CT-IgG） and Chlamydia hsp60 （chsp60-IgG） in the screening of oviduct infertility. Methods 55 serum samples of laparoscopic diagnosed tubal infertility patients and 116 samples from the normal subjects on empty stomach were collected.ELISA and immune turbidity methods were used to test for the two serological laparoscopic markers. Positive predictive value, negative predictive value, sensitivity, specific degrees, Youden and OR value of individual indicators were calculated. Results The positive rate of CT-IgG antibody in the patients and normal control subjects was 54.55% and 7.76%, respectively. The positive rate of Chsp60-IgG in the patients and control subjects was 40.00% and 22.41%, respectively. The positive rate of CT-IgG and chsp60-IgG in the patients and control subjects was 87.27% and 24.1%, respectively. The OR value of CT-IgG in these two groups was 14.3. The OR value of combined CT-IgG and chsp60-IgG detection in these two groups was 21.6.The OR values are all statistically significant. Conclusion CT-IgG and chsp60-IgG may be used clinically in predicting tubal infertility.
