摘　　要：目的应用实时三维超声心动图(real-time 3D echocardiography,RT-3DE)技术评价法洛四联症(Tetralogy of Fallot,TOF)患者手术前后左室容积及收缩功能。方法 TOF患者35例,对照组35例,按年龄将TOF患者分为儿童组和成人组。观察对照组和TOF患者术前、术后7 d及术后3个月左室舒张末期容积(LVEDV)、左室收缩末期容积(LVESV)及左室射血分数(LVEF)。结果与术前相比,TOF患者术后LVEDV、LVESV显著增加(P<0.05),术后7 d LVEF增加不明显,术后3个月显著增加(P<0.05)。与对照组相比,TOF患者术前及术后7 d LVEDV、LVESV、LVEF减低(P<0.05),术后3个月LVEDV减低(P<0.05),LVESV、LVEF差异无统计学意义。TOF患者的成人组与儿童组相比,术前LVEDV、LVESV增加(P<0.05),术后差异无统计学意义。结论 RT_3DE可定量评价TOF患者左室容积及收缩功能变化。Objective To evaluate left ventricular volume and systolic function in patients with tetralogy of Fallot （TOF） using real-time 3D echocardiography （RT-3DE）.Methods 35 patients with TOF were studied.35 healthy individuals served as controls.According to age,patients with TOF were divided into children group （24 cases） and adult group （11 cases）.Cardiac full-volume datasets were collected using RT-3DE,left ventricular end diastolic volume （LVEDV）,left ventricular end systolic volume （LVESV） and left ventricular ejection fraction （LVEF） were calculated 3 days before operation,7 days and 3 months after operation by the quantitative analysis software of RT-3DE,and compared with controls.Results Compared with pre-operation,LVEDV and LVESV increased markedly at 7 days and 3 months after operation （P＜0.05）,LVEF increased significantly at 3 months after operation （P＜0.05）.There were no statistical differences at 7 days after operation.Compared with controls,LVEDV,LVESV and LVEF decreased at pre-operation and 7 days after operation,LVEDV decreased at 3 months after operation,while there were no significant differences between LVESV,LVEF of 3 months after operation and controls.Compared with children group,LVEDV,LVESV increased before operation in adult group,while there were no significant differences at postoperation.Conclusions RT-3DE can be used as a quantitative method to evaluate left ventricular volume and systolic function.
