胃肠充盈超声造影在普查胃十二指肠病变临床应用初步报告

摘　　要：目的探讨口服超声造影(oral contrast ultrasonography,OCUS)普查正常人群胃十二指肠病变的价值。方法选取上海市崇明县社区居民621例进行OCUS普查胃十二指肠病变,让受检者快速吞饮口服超声造影剂,实时观察贲门、胃底部、胃体部、胃角、胃窦及十二指肠球部;检测贲门内径,胃体和胃窦胃壁厚度,幽门管内径,观察各部位形态及层次结构,同时记录图像质量。检出病变者行胃镜检查,与活检和/或手术病理对照。结果 OCUS普查检出各类胃十二指肠病变共9例,检出率为1.45%。检出胃癌1例,检出率为0.16%。结论 OCUS可用于普查胃十二指肠病变,OCUS初筛联合胃镜模式普查胃十二指肠病变值得进一步研究评估。Objective To discuss the value of screen on stomach and duodenum disease by oral contrast ultra sonography.Methods An oral contrast ultrasonography screening was conducted in 621 residents of Congming County in Shanghai.Subjects swallowed acoustic contrast agent and cardia,gastric fundus,body,angle,antrum,duodenal bulb were observed and recorded.To detect the cardia diameter、gastric wall thickness of gastric body and antrum,pyloric canal diameter,at the same time observing the appearance and structure.Patients then received gastroscopies and the ultrasound was compared with biopsy or operation pathology.Results Nine cases were identified at the OCUS screen and the total detection rate was 1.45 ％.We found 1 case of gastric cancer.The detection rate of gastric cancer was 0.16％.Conclusions OCUS could be used as a screen method on stomach and duodenum disease.The pattern of the screen using OCUS combining with gastroscopy had the value of further research.
