80岁以上高血压患者血压变异与左室质量指数的探讨

摘　　要：目的评价>80岁高血压患者血压变异性(blood pressure variability,BPV)与左室质量指数(left ventricular mass index,LVMI)的变化。方法入选>80岁老人155例,按照血压情况分为无高血压(normotensive,NH)组、高血压未治疗(hypertension,HT)组、高血压治疗达标(treatment,TG)组,比较其24h动态血压参数和LVMI。结果 HT组与NH组、TG组LVMI比较有统计学差异(P<0.05);HT组与NH组在日间收缩压变异率、夜间收缩压变异率、夜间舒张压变异率、24h收缩压变异率、24h舒张压变异率、24h平均压变异率及夜间平均压变异率方面比较均有统计学差异(P<0.05)。TG组与NH组比较,夜间舒张压变异率及24h舒张压变异率有统计学差异(P<0.05)。结论 >80岁老年人群血压水平决定左心室重构,同时高血压患者较无高血压者血压变异明显,尤其日间、夜间、24h收缩压变异,24h平均压变异、夜间平均压变异更为明显。Objective To investigate the changes of pressure variability and left ventricular mass index （LVMI） in hypertensive patients aged 80 and over. Methods A total of 155 patients aged 80 and over were divided into three groups according to the level of blood pressure ： normotensive（NH） group, hypertension（HT） group or treatment （TG） group. Am- bulatory blood pressure monitoring and echocardiography detection were performed in all patients. Results There was significant difference in LVMI between HT group and the other two groups.Day systolic blood pressure variability（ dSBPV）, night diastolic blood pressure variability（ nDBPV）, 24 h systolic blood pressure variability （24hSBPV）, 24 h diastolic blood pressure variability （24hDBPV）, 24 h mean blood pressure variability （24hMBPV） and night mean blood pressure variability（nMBPV） in HT group were significantly higher than those in NH group.Night diastolic blood pressure variability （ nDBPV）, and 24hDBPV in TG group were significantly higher than those in NH group. Conclusions The level of blood pressure in hypertensive patients aged 80 and over plays an important role in left ventricular remodeling, blood pressure va- riability and cardiac hypertrophy.
