系统化护理干预对老年糖尿病患者人工膝关节置换术前血糖的影响

摘　　要：目的观察系统化护理干预对老年糖尿病患者人工膝关节置换术前血糖的影响。方法行人工膝关节置换术老年糖尿病患者120例,随机分为观察组和对照组,各60例,对照组给予常规骨科护理,观察组在此基础上增加系统化护理干预,包括心理康复、认知行为干预、家庭社会支持和放松训练。分别在患者入院第2天和术前1 d采用SAS焦虑自评量表评定其焦虑程度;入院第2天及手术当天测量所有患者的空腹血糖值。结果术前1 d观察组的SAS评分为(41.19±2.22)分、对照组为(46.06±3.87)分,两组比较P<0.01。手术当日观察组空腹血糖为(7.99±1.54)mmol/L、对照组为(8.65±1.99)mmol/L,两组比较P<0.05。结论系统化护理干预对行人工膝关节置换术老年糖尿病患者的血糖控制效果优于常规护理。Objective To observe the impact of systematic nursing intervention on the preoperative blood glucose level of the elderly diabetic patients undergoing knee arthroplasty. Methods We collected the information of 120 elderly diabet- ic patients who underwent knee arthroplasty. They were randomly divided into the control group and the observation group （60 cases each）. The patients in the control group were treated with routine orthopedic care and the patients in the obser- vation group was additionally applied to the systematic nursing intervention including psychological rehabilitation, cognitive behavioral therapy, family and social support and relaxation training on the basis of routine orthopedic care. The anxiety was evaluated by anxiety self-assessment scales （SAS） on the second day on admission and one day before surgery, and the fasting blood glucose （FBG） was assessed on the second day on admission and on the day of surgery. Results The SAS score at 1 d before surgery in the observation group was （41.19 ±2.22）, and （46.06 ± 3.87）in the control group （P 〈 0.01 ）. FBG on the day of surgery in the observation group was （7.99 ±1.54） mmol/L and （8.65± 1.99） mmol/L in the control group （ P 〈 0.01 ）. Conclusion The systematic nursing intervention can more effectively control FBG than routine nursing care in the elderly diabetic patients undergoing knee arthroplasty.
