模拟推拿手法对家兔颈椎间盘倾斜角度及生物力学变化的影响

摘  要:目的:从家兔颈椎间盘倾斜角度的侧面出发,研究推拿手法与颈椎曲度、生物力学之间的关联性.方法:对家兔颈椎进行CT扫描,通过MIMICS三维重建软件将扫描图像进行三维重建,运用MIMICS三维重建软件及ANSYS有限元生物力学分析软件对图像和三维颈椎进行分析.结果:不同三维模型同一椎间盘节段倾斜角度比较:相同椎间盘节段颈屈后伸位模型与正常曲度位模型比较均有显著性差异(P<0.05),且椎间盘1、3、5、6节段颈屈后伸位模型与正常曲度位模型比较有显著性差异(P<0.01);前屈20°应力下同一颈椎节段不同曲度模型间应力比较:颈屈后伸位模型椎间盘倾斜角度虽然与正常曲度位模型比较也有显著性差异,但变化不大.结论:颈屈后伸位(即模拟推拿后伸手法)各节段椎间盘应力变化处于良性状态.Objective: To study the relationship between manipulation and cervical curvature, biomechanics from the side of inclination angle of do- mestic rabbit's cervical disc. Methods: Scanned the rabbit's cervical spine by CT, three-dimensional reconstructed the scanned images by MIMICS three-dimensional reconstruction software, then used MIMICS three-dimensional reconstruction software and ANSYS finite element biomechanical analysis software to analyze the images and three-dimensional cervical spine. Results: The comparison of inclination angles of the same disc segment in different three-dimensional models: there was significant difference between flexion and posterior extension model and normal curvature model at the same disc segment (P〈0.05), and there were significant difference between flexion and posterior extension model and normal curvature model at first, third, fifth, sixth segment (1~〈0.01); the comparison of the stress of different curvature models at the same disc segment under 20 anteflexion stress: though there was significant difference of inclination angles between flexion and posterior extension model and normal curvature model, but little changed. Conclusion: The change of the stress of disc segments at a position of flexion and posterior extension (simulated posterior extension manipulation) is at benign condition.
