摘 要:目的 探讨高胆红素血症(高胆) 早产儿血清白介素-6( IL-6) 水平的变化及其临床意义.方法选择我科住院的60例高胆早产儿(按TSB 170~256 μmol/L,257~342 μmol/L,>342 μmol/L分为轻、中、重3组)和 22例非高胆早产儿,清晨留取静脉血4 mL,取血清标本,测血清总胆红素,并采用双抗体夹心ELISA方法检测血清IL-6,血清酸性钙结合蛋白(S-100)水平,并比较组间IL-6、S-100、TSB 水平的差异及其相关性.结果 轻、中、重高胆早产儿血清IL-6水平分别为(8.56±2.56)μg/L、(12.65±2.59)μg/L、(16.43±3.67)μg/L,较对照组(6.45±1.23)μg/L明显升高,轻、中、重高胆组IL-6水平比较差异有统计学意义(P<0.01);高胆组S-100水平分别为(0.345±0.17)μg/L、(0.483±0.26)μg/L、(0.863±0.38)μg/L,明显高于对照组(0.293±0.12)μg/L(P<0.01),各组比较差异均有统计学意义(P<0.01);血清IL-6水平与TSB、S-100均呈正相关(r=0.692、0.674,P<0.05).结论 IL-6与高胆早产儿脑损伤密切相关,可作为预测高胆早产儿脑损伤轻重程度的生化指标之一.Objective To study the change of serum interleukin-6 in premature infants with hyperbilirubi-nemia and its clinical significance. Methods Sixty premature infants with hyperbilirubinemia (experiment group) and 20 premature infants without hyperbilirubinemia (control group) were selected, and they were treated in our hospital. The levels of total serum bilirubin (TSB) : 170-256μmol/L were defined as mild hyperbilirubinemia group, 257-342 μmol/L as moderate group, and 〉 342 μmol/L as severe group. Then TSB were detected and serum interleukin-6 and S-100 protein concentration were detected by double antibody sandwich ELISA method. Results The level of IL-6 in serum in mild, moderate, and severe hyperbilirubinemia groups(8.56±2.56)μg/L,(12. 65±2. 59)μg/L,(16.43±3.67) μg/L all increased more significantly than that in control group(6.45±1.23)μg/L, P〈0.01, and there were significant differences among the experiment groups(P〈0.01). The mean S-100 in experiment groups(0. 345±0. 17)μg/L, (0. 483±0.26) μg/L, (0. 863±0.38) μg/L were significantly higher than that in control group (0. 293±0.12)μg/L, and there were significant differences among experiment groups(P〈0.01). Serum interleukin-6 levels were positively correlated to S-100 and TSB (r=0. 692, 0. 674, P〈0.05). Conclusion IL-6 may be closely correlated to the brain injury of premature infants with hyperbilirubinemia; IL-6 levels can act the biochemical indicator for predicting brain injury severity in premature infants with hyperbilirubinemia.
