杭州地区680例变应性鼻炎患者常见过敏原检测与分析

摘　　要：目的了解杭州地区变应性鼻炎患者常见过敏原分布情况，为临床防治提供一定的依据。方法应用德国Mediwiss敏筛（aHergyscreen）过敏原免疫印迹定量检测系统对680例变应性鼻炎患者进行血清总IgE和过敏原特异性IgE检测。根据年龄将患者分为儿童组（2～18）岁和成年组（19—82）岁，根据性别分为男性组和女性组。比较各组在过敏原阳性率分布上的差异。结果680例变应性鼻炎患者患者，总IgE阳性578例（85．00％），其中检测出有特异性IgE阳性489例（71．91％）。阳性率最高的前三位过敏原分别为户尘螨粉尘螨（326例，47．94％）、屋尘（168例，24．71％）和狗毛皮屑（83例，12．2l％）。不同性别间，各种过敏原的阳性率差异无统计学意义（P〉0．05）；不同年龄组间，过敏原阳性率差异有统计学意义的有户尘粉尘螨、屋尘、狗毛皮屑、鸡蛋白鸡蛋黄、牛奶、苋和混合霉菌（P〈0．05）。结论杭州地区变应性鼻炎患者过敏原以螨、屋尘和狗毛皮屑为主，儿童与成人患者在过敏原分布上有所差异。Objective To understand the distribution of common allergens among allergic rhinitis （AR） patients in Hangzhou, and provide references for clinical prevention and treatment. Methods Serum total IgE （tIgE） and allergens specific IgE （sIgE） in 680 AR patients were detected by Mediwiss sensitive screening allergens quantitative detection system（ enzyme immu- noassay method）. The samples were divided into children group（2 - 18 years old） and adults group（ 19 ~ 82 years old） accord- ing to age, aas well as male group and female group according to gender. The positive rate of allergens was compared between these groups. Results The positive rates of tlgE and sIgE were 85.00% （578 cases） and 71.91% （489 cases） respectively in 680 cases. The top three allergens were dust mites（326 cases, 47.94% ）, house dust（ 168 cases, 24.71% ） and dog hair dander（83 cases, 12.21% ）. There were no significant differences in comparing the positive rate of allergens between male and female patients（P 〉 0.05）. Whereas, the positive rates of dust mites, house dust, dog hair dander, egg, milk, Amaranthus and fungus had statistical difference between children group and adult group （P 〈 0.05 ）. Conclusion In Hangzhou, the most common allergens in AR patients were dust mite, house dust and dog hair dander, and the allergens distribution has discrepancy between children and adult patients.
