吻合器痔上黏膜环切术联合消痔灵注射液治疗会阴下降综合征37例临床分析

摘　　要：目的探讨吻合器痔上黏膜环切术（PPH）联合消痔灵注射液治疗会阴下降的临床效果。方法回顾性分析2010年6月~2011年6月在沧州市中心医院肛肠外科接受PPH联合消痔灵注射治疗的37例成人会阴下降综合征患者的临床资料,观察其近远期疗效及并发症。结果 37例患者均顺利完成手术,手术时间为（40.6±5.4）min;术中失血量为（44.23±5.72）mL;住院时间为（8.2±1.3）d;术后会阴部疼痛不适症状均消失,无大出血、肛周脓肿、肛瘘、腹腔感染及泌尿生殖功能障碍等并发症发生。术后便秘症状缓解率为75.68%,排粪困难缓解率为72.97%。术后随访：随访1~12个月,平均8.24个月;37例患者进行肛门功能Kirwan评级,Ⅰ级33例（89.2%）,Ⅱ级4例（10.8%）。随访1年,1例复发,余36例预后良好。总有效率为97.3%。结论 PPH联合消痔灵注射液治疗会阴下降操作简单,疗效确切,并发症少,复发率低,患者痛苦少,是一种安全有效的治疗方法。Objective To evaluate the efficacy of procedure for prolapse and hemorrhoids（PPH） with Xiaozhiling Injection for descending perineum syndrome. Methods The clinical data of 37 adult patients were analyzed retrospectively with descending perineum syndrome accepted PPH combined with Xiaozhiling Injection in the Department of Anorectal Surgery of Cangzhou Central Hospital from June 2010 to June 2011. The near and forward curative effect and complications were observed. Results 37 patients were successfully completed operation, operation time was（40.6±5.4） minutes; intraoperative blood loss was（44.23±5.72） mL; hospitalization time was（8.2±1.3） days; postoperative perineal descent symptoms disappeared. After descending perineum symptoms disappeared, no complications such as bleeding,anal fistula, abdominal cavity infection and reproductive dysfunction. Postoperative constipation symptom relief rate was75.68%, defecation difficulties remission rate was 72.97%. Postoperative follow-up： follow-up time from 1-12 months,with an average follow-up time of 8.24 months; 37 patients with anal function Kirwan grades, 33 cases（89.2%） were grade Ⅰ, 4 cases（10.8%） were grade Ⅱ. Follow-up of 1 year, 1 case of recurrence, 36 patients prognosis were good.The total effective rate was 97.3%. Conclusion PPH combined with Xiaozhiling Injection in the treatment of descending perineum syndrome has the advantages of simple operation, definite curative effect, less complications, low recurrence rate, less pain for patients, which is a safe and effective treatment method.
