保肝降酶中药在慢性乙型肝炎治疗中的系统评价

摘　　要：目的:系统评价保肝降酶中药在慢性乙型肝炎(CHB)治疗中的疗效。方法:系统检索中国期刊全文数据库(CNKI)、中文科技期刊数据(VIP)、万方数据知识服务平台、MEDLINE、Embase和Cochran Library数据库(截至2013年5月)中有关保肝降酶中药治疗CHB的随机对照研究(randomized clinical trial,RCT),并采用Jadad法评价文献质量,对评分≥4分的19篇RCT研究结果进行合并,应用Meta分析法评价保肝降酶中药在CHB治疗中的疗效特点。结果:共纳入19项研究,涉及2554名患者。Meta分析结果显示,保肝降酶中药联合西药抗病毒治疗对促进ALT和AST复常、HBeAg转阴、HBeAg/抗HBe转换及降低HBV DNA载量等方面具有一定作用。结论:保肝降酶中药有助于恢复、稳定肝功能,促进HBeAg转阴及血清转换,降低HBV DNA载量等,在CHB治疗中具有重要作用。Objective： To evaluate the protection the liver and reduction enzyme effect of Chinese medicine to treat with chronic hepatitis B （ CHB ） . Methods： Searching the literatures of randomized clinical trial （RCT） databases in CNKI, WangFang Data, VIP, MEDLING, Embase and Cochran Library in the protection the liver and reduction enzyme effect of Chi- nese medicine to treat with chronic hepatitis B up to May, 2013, Using Jadad method to value the qualities of these papers gained from these databases and then analysed and integrated the experiment resaults from 19 papers which were above 3 score ac- cording to the revise edition of Jadad scale. Then we used meta analysis to evaluate features of Chinese medicine of protection the liver and reduction enzyme. Results： This research included 19 RCTs with 2554 patients. The resaults showed that Chinese med- icine had the effect of protection the liver and reduction enzyme to promote the recovery of ALT and AST levers, the turn negative of HBV-e antigen, the transformation of HBV-e antigen and HBV-e antibody as well as the reduction of the HBV DNA load com- bined with antiviral medicine to some extend. Conclusion： This kind of Chinese medicine is conductive to the recovery and sta- bility of hapatic function, the turn negative of HBV-e antigen, the transformation of HBV-e antigen and HBV-e antibody as well as the reduction the HBV DNA load, all of that represented Chinese medicine has an important role in treating with CHB.
